Abstract:

Úvod. Farmakorezistentní epilepsie může být účinně léčena epileptochirurgickými postupy. Resekční výkonem lze dosáhnout dlouhodobého stavu bez záchvatů u 60-70% pacientů s farmakorezistentní epilepsií spánkového laloku (TLE). Bitemporální epilepsie je jednou z příčin selhání chirurgické léčby u pacientů s TLE. Bitemporální epilepsie je zvláštním typem TLE, která je charakterizována nezávislým počátkem záchvatů v obou spánkových lalocích. Úplné bezzáchvatovosti je možno resekcí dosáhnout u 34% pacientů s bitemporální epilepsií, avšak s ohledem na vyšší riziko pooperačního kognitivního deficitu. Důležitou skutečností je, že diagnostika bitemporální epilepsie vyžaduje invazivní video-EEG monitoraci s bilaterální implantací intracerebrálních elektrod. Cílem této práce bylo zjistit, zda může být bitemporální epilepsie identifikována pomocí 1) semiologické analýzy a 2) analýzy interiktálních vysokofrekvenčních oscilací (HFO). Metody. 1) U 13 pacientů s unilaterální (uniTLE) a 13 pacientů bilaterální epilepsií spánkového laloku (biTLE) byla provedena retrospektivní komparace iktální a postiktální semiologie. Analyzovány byly pouze záchvaty registrované při předoperační video-EEG monitoraci. U všech 26 pacientů byl hodnocen výskyt iktálních motorických příznaků, periiktálních motorických příznaků, periiktálních vegetativních příznaků, časných oroalimentárních automatismů a trvání postiktální neodpovídavosti. 2) Retrospektivně byla analyzována četnost interiktálních HFO u 9 pacientů s uniTLE a 7 pacientů s biTLE, kteří v rámci předoperační evaluace podstoupili invazivní video-EEG vyšetření s implantaci intracerebrálních elektrod do obou hipokampů. Detekce tradičních interiktálních HFO, tzv. ripplů (80–250Hz) a fast ripplů (250–600 Hz) byla provedena automatizovaným způsobem. Výsledky. 1) U pacientů s biTLE byl prokázán nižší výskyt iktálních motorických příznaků a delší trvání postiktální neodpovídavosti než u pacientů s uniTLE. 2) U pacientů s uniTLE byla nalezena vyšší četnost ripplů a fast ripplů v ipsilaterálních než v kontralaterálních hipokampech (vztaženo ke straně počátku záchvatu), zatímco u biTLE nebyl pozorován rozdíl mezi oběma stranami. Neepileptické hipokampy u pacientů s uniTLE se od ostatních analyzovaných hipokampů odlišovaly signifikantně nižší četností ripplů. Závěr. Bitemporální epilepsie může být odlišena od unilaterální TLE na základě hodnocení klinické semiologie epileptických záchvatů i analýzy interiktálních HFO v hipokampech. Neinvazivní video-EEG monitorace a interiktální záznamy při invazivním EEG vyšetření mohou přispět ke zlepšení diagnostiky pacientů s bitemporální epilepsií.

Abstract:

Introduction. Drug-resistant epilepsy can be treated surgically. Resective surgery results in seizure freedom in 60-70% of patients with intractable temporal lobe epilepsy (TLE). One cause of TLE surgery failure is bilateral temporal lobe epilepsy. Bitemporal epilepsy (biTLE) is a specific type of TLE characterized by independent bilateral seizure onset in both temporal lobes. Using resective surgery, seizure freedom might be achieved in 34% of patients with biTLE, but a higher risk of postsurgical cognitive decline is possible. Notably, identifying biTLE requires invasive EEG monitoring using bilateral intracerebral electrode implantation. The main aim of this thesis was to determine whether biTLE might be identified by 1) seizure semiology analysis and 2) interictal high frequency oscillation (HFO) analysis. Methods. 1) We retrospectively compared ictal and postictal semiology of 13 patients with unilateral temporal lobe epilepsy (uniTLE) and 13 patients with biTLE. For the analysis, only presurgical video-EEG recordings were used. In all 26 patients, the frequency of predefined clusters of ictal and periictal signs were evaluated: ictal motor signs, periictal motor signs, periictal vegetative signs, the frequency of early oroalimentary automatisms, and the duration of postictal unresponsiveness. 2) Interictal HFO rates were evaluated retrospectively in 9 patients with uniTLE and in 7 patients with biTLE. All patients underwent bilateral hippocampal depth electrode implantation as a part of preoperative evaluation. Interictal HFO, ripples (80–250Hz) and fast ripples (250–600 Hz), were detected automatically. Results. 1) Patients with biTLE have a lower frequency of ictal motor signs and longer duration of postictal unresponsiveness than patients with uniTLE. 2) In uniTLE, higher ripple and fast ripple rates were found in the hippocampi ipsilateral to the seizure onset than in the contralateral hippocampi; there were no differences between the two sides in biTLE. Non-epileptic hippocampi in uniTLE were distinguished by significantly lower ripple rates than in the remaining analysed hippocampi. Conclusions. Bitemporal epilepsy might be distinguished from uniTLE by both seizure semiology and interictal hippocampal HFO analysis. Non-invasive video-EEG monitoring and interictal recordings during invasive EEG might contribute to the improvement of bitemporal epilepsy identification.
