Dynamic study of women´s pelvic floor with pelvic organ prolapse

Abstract:

Tématicky je tato práce členěna do několika částí. V úvodu jsem se zaměřila na popis anatomie pánevního dna (a to pohledem významným pro urogynekologii) a také na fyziologii funkce v této oblasti. Další oddíl obsahuje popis jednotlivých vyšetřovacích technik, které je možné využít při stanovení diagnózy onemocnění. U zobrazovacích metod se zmiňuji o základním principu dané techniky, v případě magnetické rezonance uvádím i vyšetřovací protokol používaný na našem oddělení. Následující části definují prolaps orgánů malé pánve a představují možnosti klasifikace prolapsu pánevních orgánů a možné terapeutické postupy. Je zde výčet jednotlivých možností léčby. Uvedeny jsou metody konzervativní i operační. Volba operační techniky se liší podle postižení jednotlivých oddílů - kompartmentů. V závěrečné části uvádím výsledky, které jsem získala sběrem dat za dané časové období (forma retrospektivní studie). Z těchto zpracovaných dat jsem nabyla dojmu, že vyšetření magnetickou rezonancí je přínosnou metodou nejen při prvním vyšetření pacientky, ale zejména při stagingu po operačním řešení, kdy nám pak tato metoda umožňuje zobrazit stupeň regrese prolapsu pánevních orgánů.

Abstract:

The present assignment is topically divided into several parts. The initial part focuses on the description of the anatomy of pelvic floor (from the point of view important for urogynaecology) and also on the physiological function of the given area. The following part comprises a description of individual examination techniques that can be used to diagnose diseases. For the depictive methods a basic principal of each method is mentioned. In case of magnetic resonance a protocol used at our department is described. The following parts define the prolapse of the lesser pelvis organs and introduce possibilities of the classification of pelvic organ prolapse and possible therapeutic procedures. Separate possibilities of treatment are listed and conservation as well as surgery methods are also mentioned. Selection of the most suitable surgery method varies depending on how separate sections - compartments - are affected. The closing part deals with the results achieved from a data collection performed during a selected period of time (in the form of a retrospective study). The processed data reveal that the magnetic resonance examination is a contributing method not only at the first examination of a patient but especially at the staging after a surgery, when the given method makes it possible to visualize the degree of the regression of the pelvic organ prolapse.
