Analysis of chromosomal abnormalities using whole genome profiling methods in patients with DiGeorge syndrom

Abstract:

DiGeorgeův syndrom je nejčastější mikrodeleční syndrom s incidencí 1:4000 způsobený mikrodelecí v oblasti 22q11. Prvním cílem diplomové práce bylo zhodnocení záchytu mikrodelece 22q11 a analýza klinických příznaků u souboru 1558 pacientů vyšetřených na OLF FN Brno pro podezření na DiGeorgeův syndrom metodami FISH a MLPA® v letech 1997 až 2015. Celkový záchyt pacientů s mikrodelecí 22q11 cílenou diagnostikou metodami FISH a MLPA® činí 4,8 %. Bylo detekováno 75 pacientů s mikrodelecí 22q11, u 43 z nich byl vyhodnocen i původ mikrodelece (81 % de novo a 19 % rodičovský původ) a fenotyp (kardiovaskulární vady (69 %), faciální stigmata (69 %), poruchy řeči (28 %), defekty močové soustavy (11 %), s menší než 10% četností jsou zastoupeny snížená inteligence, imunodeficience a hypokalcémie). Druhým cílem byla celogenomová analýza metodou komparativní genomové hybridizace na DNA mikročipech (aCGH) u 24 pacientů s prokázanou mikrodelecí 22q11, na základě které byly detekovány mikrodelece 22q11 s velikostí od 1,3 Mb do 2,9 Mb s mediánem 2,5 Mb. Korelací genotypu (velikosti mikrodelece) s fenotypem pacientů byl v našem souboru objeven vztah mezi mikrodelecí 22q11 o velikosti 1,3-1,4 Mb a absencí kardiovaskulárních vad. Pro další fenotypové znaky korelace objevena nebyla, výsledky práce tak podporují již provedené výzkumy o mikrodeleci 22q11.

Abstract:

DiGeorge syndrome is the most common microdeletion syndrome with a frequency of approximately 1:4000 and it is caused by hemizygous microdeletion on chromosome 22q11. The first aim of this diploma thesis was to determine the detection rate of microdeletion 22q11 and to analyse clinical phenotypes among the 1558 patients tested by FISH or MLPA® from December 1997 to December 2015 (Oddělení lékařské genetiky Fakultní nemocnice Brno). The general positive detection rate for specific molecular diagnostic was 4.8 %. The microdeletion 22q11 was found in 75 patients and we were able obtain the microdeletion origin data of 43 individuals (81 % de novo microdeletions and 19 % familiar origin). The clinical findings seen in patients are cardiac abnormalities (69 %), facial dysmorphology (69 %), speech delay (28 %), renal abnormalities (11 %), and with lower frequency than 10 % developmental delay, immunodeficiency and hypocalcaemia. The second aim was the whole-genome analysis in 24 subject with microdeletion 22q11 performed by array comparative genome hybridization (aCGH). Whole-genome aCGH detected a 1.3 Mb to 2.9 Mb (median = 2.5 Mb) microdeletion at 22q11. Genotype (size of microdeletion) 1.3-1.4 Mb correlated with absence of cardiac abnormalities in our cohort of patients. However the correlation was not found for the other clinical phenotypes. Our results support the previous studies knowledge about microdeletion 22q11.
