Alternate fractionation schedules

Abstract:

Současné techniky zevní radioterapie neumožňují aplikaci záření pouze do nádorového ložiska, ale dochází k ozáření i okolní zdravé tkáně. Abychom nepoškodili kritické orgány, je potřeba dávku záření rozdělit do několika malých částí ? frakcí. Referenčním frakcionačním režimem je tzv. normofrakcionace, v určitém procentu případů je však výhodnější použít jiné frakcionační režimy, zvané alternativní frakcionační schémata. Bakalářská práce je rozdělena na část obecnou a část speciální (tzn. vlastní studie). Obecná část je zaměřena na alternativní frakcionační schémata, na klinické využití, výhody a nevýhody těchto frakcionací. Vlastní studie popisuje akutní toxicitu u ozařování Martinezovou technikou (velkoobjemové ozáření protrahovanou frakcionací).

Abstract:

Current radiotherapy techniques aren?t capable to limit the therapeutic irradiation to the target volume. The surrounding healthy tissue becomes a remarkable part of dose as well. At present time, we only are able to minimize the toxicity by dividing the dose of radiation into several small parts ? fractions. The reference fractionation regimen is called classical fractionation and is used by vast majority of patients. In some patients, however, we can take advantage of using other fractionation regimens, called alternative fractionation schedules. The thesis is divided into two parts, a theoretical part and a clinical study. The theoretical part aims at alternate fractionation schedules in general, their clinical use, advantages and disadvantages. The clinical study describes the acute toxicity of Martinez technique (large volume protracted fractionation radiotherapy).
