Continuity of Safety in Health Care in General Practitioner's Office

Abstract:

Abstrakt Cílem této práce byla analýza informací o kontinuitě bezpečnosti ve zdravotnictví, spolupráce mezi ordinacemi praktických lékařů a zdravotních služeb v terénu, kde ohniskem problematiky byla ambulantní zdravotní péče. Dalším cílem bylo nalézt evaluaci zlepšení léčebného procesu u pacientů. V diplomové práci byl použit kvalitativní výzkum řízeným rozhovorem a na základě dotazníku o bezpečnosti a spolupráci mezi ordinacemi praktických lékařů a zdravotního personálu v terénu. Respondentů bylo celkem osm. Pět praktických lékařů s vlastní soukromou praxí a tři zdravotní pracovnice v terénních zdravotních službách na vedoucích pozicích. Dále byla provázena SWOT – analýza vlastního podniku zdravotní terénní peče. Kvalitativní výzkum ukázal, že názory v ordinacích a v terénu se vůči bezpečnosti celkem neliší. Všichni dotázaní lékaři i všechny pracovnice terénní služby vedli elektronické záznamy o zdravotním stavu pacientů. Lékaři uvedli, že elektronické záznamy jsou přístupné zdravotním pracovníkům v terénu, jenže pracovnice v terénu tvrdily, že jim jsou záznamy jen částečně dostupné. Stejně tak, že zdravotní personál dostává od nich zpětnou vazbu ohledně péče o pacienty a že podporuje zdravotní personál v otázce ohlášení nehody či nežádoucí události. Ale jen 33.3 % zdravotního personálu v terénu s tímto tvrzením souhlasilo. V evaluaci a zdokonalování léčebného procesu používají lékaři jiné informace než zdravotní personál. Bilance SWOT-analýzy podniku byla celkem pozitivní. Největší hrozbou je nedostatek zdravotního personálu a dále pak hrozby týkající se ekonomických a ekologických změn. Výzkumná práce ukázala, že v budoucnu by se mělo zdravotnictví České republiky více zaměřit na vzájemnou spolupráci všech zařízení na co nejvyšší úrovni, a to v rámci aktuálně dané legislativy.

Abstract:

One aim of this thesis was the analysis of the information on the continuity of safety in the health sector, cooperation between general practitioners and health services in the field, where the focus of the issue was home health care. Another aim was to find the evaluation of the improvement of the treatment process for patients. In this thesis there was used a qualitative research method in the form of an interview on the basis of a questionnaire on safety and cooperation between general practitioners and health personnel in the home care field. There were a total of eight respondents, five of whom owned a private practice and three senior position health services workers. Furthermore, the methods were accompanied by a SWOT – analysis of my own business in the home health care sector. The qualitative research showed no main difference regarding the safety of the health care workers in offices and in the field. Both doctors and health care workers in the home care sector kept electronic records of their patients. Doctors pointed out that electronic records are accessible to the health workers in the home care sector. Yet the medical staff in the home care sector stated that records were only partially available to them. Further, doctors stated that they give feedback to the medical staff in the home care sector and that they support the medical staff in reporting an accident or incident. But only 33 % of the medical staff in the home care sector agreed with this statement. Physicians use information other than health care professionals to evaluate and improve the treatment process. The balance of the SWOT analysis of the company was quite positive. The greatest threats here are the lack of health personnel and the threat of economic and environmental changes. This thesis has shown that in the future the health care of the Czech Republic should focus more on the mutual cooperation of all facilities at the highest possible level, within the actual legislative framework.
