Breastfeeding of a premature newborn

Abstract:

Tato práce se zabývá kojením nedonošených novorozenců. Cílem bylo shromáždit takové informace, které vysvětlí určité odlišnosti od kojení novorozenců narozených v termínu a zjistit míru informovanosti matek předčasně narozených dětí o tomto problému. V teoretické části je vysvětlen termín nedonošený novorozenec, anatomie prsu a fyziologie laktace. Dále již je práce zaměřena na odlišnosti kojení nedonošených novorozenců - vývoj sání, polykání, pasáže, trávení a resorpce živin, zvláštnosti výživy, význam mateřského mléka a výhody kojení novorozenců s nízkou porodní hmotností. Práce se také věnuje určitým zvláštnostem kojení, které je třeba respektovat a znát u nedonošených novorozenců, zásadám správné techniky a vhodným polohám při kojení dříve narozených dětí. Neopomněla jsem uvést ani důležitost odstříkávání mateřského mléka, alternativní způsoby krmení, ale i překážky a problémy se kterými se matka a zdravotníci mohou setkat, a to jak ze strany dítěte, tak i ze strany matky. Pár řádků na konec této práce jsem věnovala metodě kangaroo mother care, která se pomalu dostává do popředí zájmu jak lékařů, tak i laiků zejména při zahájení a udržení laktace. Podkladem pro praktickou část se stal výzkum zaměřený na informovanost matek o kojení nedonošených novorozenců, který byl prováděn dotazníkovou metodou ve 2 brněnských nemocnicích. Z analýzy výsledků byly zjištěny nedostatky v informovanosti o dané problematice, a tak byla vytvořena edukační brožurka.

Abstract:

The thesis considers primarily the issue of prematurely born babies, particulary their breast feeding. The main goal was to find out all the information which will explain certain differencies between breast feading of prematurely born and normaly born infants and find out the level of awarness and knowledge from mothers of prematurely born infants. In the theory part there is explained the term “ prematurely born infant”, breast anatomy and physical lactation. Futher the thesis is aimed on differencies between breast feeding of prematurely born infants-evolution of sucking, swallowing, digestion, resorption of nutrients, abnormalities of feeding, purpose of breast milk and advantagies of breast feeding of prematurely born infants with low weight at birth. The other part is dedicated to certain singularities of breast feeding (which should be known by premature born infants) principles of right positions and technics by breast feeding for premature born children. I have not forgotten to mention the significance of breast milk squirting, alternative ways of breast feeding or the inconveniences which can encounter the mother or nurse. The conclusion of this thesis is dedicated to the method called “kangaroo mother care” which is steadily getting into the interest of both, doctors and nonspecialists, especially during the beginning and sustainment of lactation. The Background for practical part is formed by research of mother awarness about breast feeding of premature born infants. There were collected from questionnaires of two hospitals established in Brno. There results from analyses confirmed that there is a lack of information for this issue, so as a final consequence there was an information brochure created.
