Immune-related adverse events after administration of checkpoint inhibitors

Abstract:

Rigorózní práce se zabývá imunoterapií checkpoint inhibitory a jejími imunitně podmíněnými nežádoucími účinky. V teoretické části detailně popisuje imunitní systém a nádory, nádorové antigeny, imunoeditaci nádoru, mechanismy úniku nádoru z imunitního dohledu, kontrolní body imunitního systému a také další možnosti protinádorové imuno-terapie. Více pozornosti je dále věnováno monoklonálním protilátkám jako inhibitorům kontrolního bodu imunitního systému, konkrétně an-ti-CTLA-4 a anti-PD-1/PD-L1. Několik kapitol se zaměřuje na mecha-nismy vzniku imunitně podmíněných nežádoucích účinků, specifika jejich výskytu, možnou léčbu a na prediktivní biomarkery. Na pěti po-pisných kazuistikách je pak ilustrován nástup, průběh a management těchto imunitně podmíněných nežádoucích účinků. Teoretické poznat-ky o výskytu a rizicích nežádoucích účinků checkpoint inhibitorů jsou tak dány do klinických souvislostí.

Abstract:

The rigorous thesis deals with immunotherapy with checkpoint inhibi-tors and immune-related adverse events. The theoretical part de-scribes the immune system and tumours, tumour antigens, tumour immunoeditation, the mechanisms of tumour escape from immune surveillance, the immune checkpoints and other options for anti-tumour immunotherapy in more detail. Monoclonal antibodies as im-mune checkpoint inhibitors, namely anti-CTLA-4 and anti-PD-1/PD-L1, receive more attention as well as their toxicity. Several chapters focus on the mechanisms of development of immune-related adverse events, the specifics of their occurrence, possible treatment and pre-dictive biomarkers. The onset, progress and management of these im-mune-related adverse events is illustrated in five descriptive case re-ports. Theoretical knowledge about the occurrence and risks of side effects of checkpoint inhibitors is thus placed in the clinical context.
