Metabolický syndrom jako rizikový fakktor u pacientů s náhradou renální funkce se zaměřením na období po transplantaci ledviny

Metabolic syndrome as a risk factor in patients on renal replacement therapy focusing on the period after kidney transplantation

Abstract:

Cílem práce bylo hodnocení prevalence metabolického syndromu a jeho komponent u pacientů po transplantaci ledviny a analýza jejich vlivu na funkci štěpu a albuminurii. Pacienti po transplantaci mají vyšší riziko rozvoje metabolického syndromu vlivem imunosupresivní léčby. Kortikosteroidy a kalcineurinové inhibitory mohou indukovat dyslipidémii a vznik diabetu mellitu. V naší práci jsme sledovali 69 pacientů po transplantaci kadaverózní ledviny. Prevalence metabolického syndromu byla v našem souboru 61,3% 3 roky po transplantaci. Prevalence potransplantačního diabetu mellitu byla 27% a abdominální obezity 59,7%. Věk pacientů s metabolickým syndromem byl nesignifikatně vyšší, věk příjemců ledviny s potransplantačním diabetem mellitem byl statisticky významně vyšší. Počet komponent metabolického syndromu negativně koreloval s funkcí štěpu. Statisticky významný byl vliv hypertrigyceridémie a posttransplatačního diabetu mellitu na dysfunkci štěpu. Chronickou dyfsunkci štěpu signifikantně predikovaly věk dárce, opožděná funkce štěpu a rejekce. Počet komponent metabolického syndromu, snížená hladina HDL cholesterolu, hypertriglyceridémie a hyperurikémie 6 měsíců po transplantaci predikovaly sníženou funkci štěpu 3 roky po transplantaci. Hyperurikémie se jako jediná ukázala prediktorem dysfunkce štěpu nezávisle na věku dárce, opožděné funkci štěpu a rejekci. Metabolický syndrom a systolický krevní tlak korelovaly s albuminurií, zvýšené hladiny celkového cholesterolu a apolipoproteinu B predikovaly vznik albuminurie. Analyzovali jsem kardiologické parametry u pacientů po transplantaci ledviny. U mladších pacientů vedla transplantace ledviny ke zlepšení kardiovaskulární rezervy, u pacientů nad 50 let věku nebylo ovlivnění těchto parametrů patrné Časná detekce metabolických poruch a dietní a terapeutická intervence u pacientů po transplantaci ledviny jsou důležité v prevenci chronické dysfunkce štěpu.

Abstract:

The aim of the study was to assess the prevalence of the metabolic syndrome and its components in kidney transplant recipients and analyse their influence on allograft function and albuminuria. The use of immunosuppressive therapy negatively influences the metabolic profile. Corticosteroids and calcineurin inhibitors may cause new-onset diabetes after transplantation and alter lipid profile. In the study we monitored 69 patients after cadaveric kidney transplantation. The prevalence of the metabolic syndrome in our study was 61.3% 3 years after kidney transplantation. The prevalance of new onset diabetes mellitus after transplantation was 27% and that of abdominal obesity 59.7%. The age of kidney transplant recipients with the metabolic syndrome was higher but not significantly. The age of kidney transplant recipients with new onset diabetes mellitus after transplantation was significantly higher. The number of the components of the metabolic syndrome was negatively correlated with the graft function. The influence of hypertriglyceridaemia and new onset diabetes mellitus after transplantation on graft dysfunction was statistically significant. Chronic allograft dysfunction was significantly predicted by donor age, delayed allograft function and its rejection. The number of components of the metabolic syndrome, low level of HDL-cholesterol, hypertriglyceridaemia, and hyperuricaemia 6 months after kidney transplantation predicted lower glomerular filtration 3 years after transplantation. Hyperuricaemia was the only significant predictor of allograft dysfunction 3 years after kidney transplantation, independently of the presence of rejection episodes, delayed allograft function, and donor age. The metabolic syndrome and systolic blood pressure correlated with albuminuria. Albuminuria was predicted by higher levels of apolipoprotein B and total cholesterol. We analysed some cardiologic parameters. Kidney transplantation is in younger patients the best method of renal replacement therapy with kidney function recovery associated with improvement of cardiovascular parameters, but in patients older than 50 years this effect was not seen. Early detection of metabolic abnormalities and dietary and therapeutic intervention in kidney transplant recipients may help to prevent chronic allograft dysfunction.
