The physiotherapy protocol following reconstruction of the acetabular edge of the hip with consequent nerve impairment: a clinical case

Abstract:

A large number of athletes, even at different levels of competitiveness, are affected, during their sports performances, by the onset of pain in the hip and functional disabilities related to the femoro-acetabular impingement and surgery is one of the options for the management of symptoms and physio-pathology (Casartelli 2018). In this paper, the rehabilitation program following arthroscopic surgery was treated, also considering the scientific data researched regarding the possibility of returning to sport and to the performance levels prior to the injury. The case of M.B., a professional footballer, who was diagnosed with detachment of the acetabular border following a traumatic event was investigated. In particular, the various rehabilitation phases to which he will undergo in order to fully recover from the injury will be studied in depth. Everything will begin with an introduction of the anatomical components involved (coxo-femoral joint, muscles, ligaments and related bone components) and an in-depth study on arthrocinematics and on the factors of coactation and stability of the hip joint. Subsequently, the femoro-acetabular impingement syndrome will be presented and the different types of possible treatment will be compared. In particular, the methods and objectives of post-surgical rehabilitation treatment and of a more conservative approach with respect to the pathology will be described, based on appropriate studies of scientific evidence that are not too remote. Finally, the treatment plan followed by the patient in the case in question and the results obtained during his journey will be illustrated. The objective of the study is to demonstrate, through the comparison of different sources and scientific studies, the importance of an adequate rehabilitation program, customized according to the patient, and a proactive approach / coping of the latter in front of the pathology and difficulties encountered

Abstract:

Un gran numero di atleti, anche a diversi livelli di competitivit?, risente, durante le loro prestazioni sportive, dell'insorgenza di dolori a livello dell'anca e di disabilit? funzionali legate all'impingement femoro-acetabolare e la chirurgia rappresenta una delle opzioni per la gestione dei sintomi e della fisio-patologia (Casartelli 2018). In questo elaborato, ? stato trattato il programma di riabilitazione in seguito ad intervento artroscopico considerando inoltre i dati scientifici ricercati riguardo alla possibilit? di ritorno allo sport e ai livelli prestativi precedenti all'infortunio. ? stato approfondito il caso di M.B., calciatore a livello professionistico, a cui ? stato diagnosticato il distacco del bordo acetabolare in seguito a un evento di tipo traumatico. In particolare verranno approfondite le varie fasi riabilitative cui si sottoporr? al fine di rientrare completamente dall'infortunio. Il tutto inizier? con un'introduzione delle componenti anatomiche interessate (articolazione coxo-femorale, muscoli, legamenti e componenti ossee connesse) e un approfondimento sull'artrocinematica e sui fattori di coattazione e di stabilit? dell'articolazione dell'anca. Successivamente verr? presentata la sindrome da impingement femoro-acetabolare e verranno confrontate le diverse tipologie di trattamento possibili. In particolare, verranno descritte le metodiche e gli obiettivi del trattamento riabilitativo post-chirurgico e di un approccio pi? conservativo rispetto alla patologia, ricercati su opportuni studi di evidenza scientifica non troppo remoti. Per finire saranno illustrati il piano di trattamento seguito dal paziente nel caso in questione e i risultati ottenuti durante il suo percorso. L'obiettivo dello studio effettuato ? quello di dimostrare, attraverso il confronto di diverse fonti e studi scientifici, l'importanza di un adeguato programma riabilitativo, personalizzato in base al paziente, e di un approccio/coping propositivo di quest'ultimo di fronte alla patologia e alle difficolt? incontrate.
