Vybrané patofyziologické aspekty poruch glukózové tolerance v těhotenství a po porodu

Selected pathophysical aspects of impaired glucose tolerance in pregnancy and postpartum

Abstract:

Úvod Gestační diabetes (GDM) je jednou z nejčastěji se vyskytujících komplikací těhotenství. Ženy s GDM mají nejen vyšší frekvenci porodnických komplikací ale také vyšší riziko vzniku diabetu po porodu či kdykoliv v pozdějším věku. Dizertační práce se zaměřuje na (i) studium možnosti identifikace rizikových faktorů/markerů přispívajících k vzniku a přetrvání porušené glukózové tolerance či diabetu u žen s anamnézou GDM v období po porodu a (ii) objasnění příp. etiopatogenetické heterogenity GDM přispívající k různému postpartum vývoji glukózové tolerance. Metody Metodologicky bylo využito následujících přístupů: retrospektivní cross-sectional „single-centre“ epidemiologické analýzy dat nemocničního informačního systému, regresního modelování s využitím parametrů glukózového metabolizmu z období 2. trimestru a postpartum u kohorty pacientek, které absolvovaly kompletní gestační a poporodní screening („single-centre“) a studií typu „případy-kontroly“ s event. „nested“ analýzami podskupiny GDM s ohledem na postpartum metabolický stav. Všechny studie zahrnovaly dohromady n = 2768 subjektů. Výsledky Pilotní studie zjistila, že prevalence perzistence glukózové intolerance po porodu se pohybuje okolo 10 % a ženy s perzistencí glukózové intolerance měly v oGTT testu absolvovaném ve 2. trimestru gravidity vyšší hodnoty glykémie. Navazující analýza prokázala, že riziko vzniku postpartum poruchy rostlo úměrně počtu abnormálních hodnot glykémie v oGTT testu. Analýza dat „case-control“ studie ukázala u žen s GDM prekoncepčně vyšší STK, BMI a peripartálně vyšší incidenci indukovaných porodů. Peripartální komplikace se dále významně častěji vyskytovaly u podskupiny GDM pacientek s obezitou, těžší formou diabetu s potřebou inzulinu či hypertenzí. Riziko konverze představovala vyšší glykémie nalačno v oGTT testu ve 2. trimestru těhotenství, obezita, hypothyreóza a kouření. Výzkum vybraných mikronutrientů ukázal přetrvávající nižší hladiny vitaminu D po porodu u žen s GDM oproti zdravým těhotným. Dále jsme nalezli nižší hladiny tiaminu u žen s GDM při adjustaci na BMI. Prokázali jsme také signifikantně vyšší hladiny CML adjustovaného na BMI u žen s GDM. Analýza zaměřená na potravinové preference ukázala, že ženy s GDM mají významně odlišné frekvence příjmu některých kategorií (ve prospěch konzumace masa, mléčných výrobků a sladkých nápojů, naopak v neprospěch syrové zeleniny) ve srovnání s ženami s těhotenstvím bez GDM. Závěr Identifikovali jsme několik faktorů, které mohou přispívat k rozvoji GDM, případně mohou být využitelné pro predikci rizika periporodních komplikací a eventuálního přechodu GDM do permanentního diabetu či porušené glukózové tolerance po porodu. Výsledky naznačují rozdíly v metabolických drahách vybraných mikronutrientů u žen s GDM oproti ženám s fyziologickým těhotenstvím. Jejich patofyziologickou roli v souvislosti s konverzí GDM do permanentní poruchy je třeba ozřejmit na větším souboru žen s GDM a delším follow-up.

Abstract:

Background Gestional diabetes mellitus (GDM) is one of the most frequent complications of pregnancy. Woment with GDM have an incerased frequency of perinatal complications and an increased risk of persistent glucose intolerance or diabetes early postpartum or anytime in later life. The thesis collates findings of recent studies aiming at (i) study of possibility of stratification of women with GDM based on the risk of persistence or re-occurrence of impaired glucose tolerance or diabetes early postpartum and (ii) understanding the possible ethiopatogenetic heterogenity of GDM contributing to different postpartum development of glucose tolerance. Methods Methodological approach consisted of rretrospective cross-sectional “single-centre“ epidemiological analysis of data from hospital information system, regression models using parameters of glucose metabolism in second trimester and postpartum in patients who completed gestational and postpartum screening (“single-centre“) and case-control study (complemented with event. “nested” analyses of GDM subgroup with known metabolic status postpartum). Entire study sample comprised n = 2768 subjects in total. Results A pilot study showed prevalence about 10 % of persisting glucose introlerance after delivery. Moreover, women with persisting glucose intolerance have had higher mid-trimester oGTT values. Subsequent analysis demonstrated the risk of pospartum glucose intolerance gradually increased with a number of abnormal values in oGTT test. Case-control study revealed a higher systolic blood pressure and BMI before pregnancy and increased incidence of induced deliveries in woment with GDM compared to pregnancies withnout GDM. Complications during delivery were even more frequent in subgroup of GDM patients with obesity, hypertension or those requiring insulin therapy. Risk of conversion was connected to increased fasting glycaemia in mid-trimester oGTT test, obesity, hypothyreosis and smoking. Study of selected micronutrients found decreased vitamin D levels after delivery in women with GDM compared to controls. We also detected lower tiamin levels adjusted to BMI in women with GDM. Finally, we found significantly higher carboxy-methyl lysin levels adjusted to BMI in GDM women. Analysis focusing on dietary preference showed significantly different eating patterns and habits in women with GDM compared to controls (incl. poultry, pork, smoked meat, dairy products, sweet beverages, and fresh vegetables consumption). Conclusions We identified several factors likely contributing to GDM development or indicating a higher peripartal risk and, at the same time, possibly conveying predictive potential for assessment of the risk of an eventual conversion of GDM to permanent diabetes or impaired glucose tolerance postpartum. Results indicate possible differences in metabolic pathways of selected micronutrients in women with GDM compared to those with physiological pregnancy. The pathophysiological role of those pathways relating to GDM conversion to permanent glucose intolerance requires further study of a large cohort of women with GDM and a longer follow-up.
