Abstract:

LAKOMÝ Radek. Prognostické a prediktivní faktory u glioblastoma multiforme. Brno, Lékařská fakulta MU, 2011, 81 s. Disertační práce. Disertační práce podává přehled o aktuálních prognostických a prediktivních faktorech u glioblastomu. Statistickou analýzou vlastního souboru pacientů s glioblastomy léčených pooperační chemoradioterapií s temozolomidem bylo provedeno zhodnocení klinických a laboratorních markerů. Pooperační stav fyzické zdatnosti, rozsah resekce, úspěšné podání plánované chemoradioterapie, chirurgické řešení recidiv u vybraných pacientů, metylační stav MGMT genu, ale také miR-21, miR-181c, miR-195 a miR-196b korelují s přežitím pacientů s glioblastomy. Mimo jiné z práce vyplývá, že kombinací miR-181c a miR-21 je možné s 92 % senzitivitou a 81 % specificitou označit skupinu pacientů s rizikem časné progrese onemocnění po resekčním výkonu.

This dissertation describes actual possible prognostic and predictive factors in glioblastoma multiforme. A statistical analysis of own cohort of patients treated with postoperative radiotherapy plus concurrent and adjuvant chemotherapy with temozolomide brings an assessment of clinical and laboratory markers. Postoperative performance status, extent of resection, successful administration of the majority of planned concurrent chemoradiotherapy, possibility of surgical treatment at the time of recurrence, methylation status of MGMT and also miR-21, miR-181c, miR-195, and miR-196b correlate with prognosis of glioblastoma patients. Above all, we suggested that the combination of miR-181c and miR-21 could be a very sensitive and specific test to identify patients at high risk of early progression after surgery.
