The multidisciplinary approach for the rehabilitation of Duchenne muscular dystrophy: case report

Abstract:

Progressive Muscular Dystrophy Xp21 is a rare genetic disease and affects males only except in rare cases, caused by mutations of the dystrophin gene localized on the X chromosome, in the Xp21. The first symptoms appear around the age of three, the disease shows up with muscle weakness, affected children have difficulty in running, getting up from the ground and climbing stairs. The clinical course is highly variable, with severe forms with early onset and rapid progression. If not diagnosed early, the disease advances rapidly involving the muscles and therefore the loss of walking around the age of twelve. Lifespan has now doubled from a few decades ago. A multidisciplinary therapeutic approach is essential for this pathology. The clinical case described in this work is that of a young adult patient suffering from DMD, who is not walking, has lumbar scoliosis, joint limitations, severe foot deformities in equinism, mid-grade respiratory fatigue.

Abstract:

La Distrofia Muscolare progressiva Xp21 consiste in una malattia genetica rara e colpisce esclusivamente i maschi, tranne che in rarissimi casi, causata da mutazioni del gene della distrofina localizzato sul cromosoma X, in sede Xp21. I primi sintomi compaiono verso i tre anni, la malattia si presenta con debolezza muscolare, i bambini che ne sono affetti presentano difficolt? nel correre, nel rialzarsi da terra ed a salire le scale. Il decorso clinico ? molto variabile, con forme gravi a insorgenza precoce e rapida progressione. Se non diagnosticato precocemente, la malattia progredisce rapidamente coinvolgendo la muscolatura e quindi la perdita della deambulazione intorno ai dodici anni. L'aspettativa di vita ad oggi ? raddoppiata rispetto a qualche decennio fa. Per questa patologia ? indispensabile un approccio terapeutico multidisciplinare. Il caso clinico descritto in questo lavoro ? quello di un paziente giovane adulto affetto da DMD, non deambulante, presenta scoliosi lombare, limitazioni articolari, deformit? severe ai piedi in equinismo, affaticamento respiratorio di grado medio.
