Characteristic of genetic profiles of patients with diabetes melitus type 1

Abstract:

Diabetes Mellitus 1. typu (DM1) patří v současné době k nevyléčitelným autoimunitním onemocněním, které značně zvyšují morbiditu a často vedou k předčasné smrti pacientů z důvodů kardiovaskulárních a metabolických komplikací. Ke vzniku DM1 dochází nejčastěji u geneticky predisponovaných jedinců, kteří nesou na povrchu svých jaderných buněk antigeny hlavního histokompatibilního systému HLA DR3 (human leucocyte antigens) a HLA DR4. Byly objeveny vysoko rizikové alely přispívající ke vzniku onemocnění a nízko rizikové alely, které mají protektivní charakter. Pro detekci HLA alel se v současné době nejčastěji používají různé metody jako je sérologická typizace, sekvenování, PCR (polymerase chain reaction), RFLP (restriction lenght fragment polymorphism) nebo mikročipy. Všechny metody jsou ekonomicky i časově náročné a pro účely rychlého screeningu relativně malého počtu alel tedy nevhodné. Cílem mé diplomové práce je vývoj nové metody pro detekci 7 specifických alel HLA asociovaných s DM1. Detekovány měly být vysoko rizikové alely DQA1*05:01, DQB1*02:01, DRB1*04:03 a DQB1*03:02 a dále nízko rizikové alely DQB1*03:01, DQB1*06:03 a DQB1*06:02. Ke specifickému určení jednotlivých alel byla zvolena metoda real-time PCR s využitím kombinace sond s LNA modifikací. Vyvinutá metoda je rychlá, méně ekonomicky náročná a snižuje riziko kontaminace.

Abstract:

Diabetes Mellitus Type 1 (DM1) is currently incurable autoimmune disease that significantly increases morbidity and often leads to premature death of patients due to cardiovascular and metabolic complications. DM1 mostly develops at genetically predisposed individuals who carry major histocompatibility antigens HLA-DR3 (human leucocyte antigens) and HLA DR4 on their nucleated cells surfaces. Scientists have discovered high-risk alleles contributing to disease suspectibility and the low-risk alleles that are protective. Nowadays, for detection of HLA alleles different methods such as serological typing, sequencing, PCR (polymerase chain reaction), RFLP (restriction lenght fragment polymorphism) and microarrays are used most frequently. All the methods are economically demanding and time consuming and therefore inappropriate for rapid screening of relatively small number of alleles. The aim of my thesis is a new method development for detection of 7 alleles associated DM1 by using of real-time PCR. High-risk DQA1*05:01, DQB1*02:01, DRB1*04:03 and DQB1*03:02 alelles should have been detected. Low risk DQB1*03:01, DQB1*06:02 and DQB1*06:03 alleles should have been detected too. The combination of primers and probes with LNA modifications were used for specific identification of the individual alleles. The developed real-time PCR method is rapid, less economically demanding and it reduces the risk of contamination.
