Geographical Attributes of HIV/AIDS Pandemic: the Collapse in the Endangered Regions, Threats for Central and Eastern Europe

Abstract:

Předkládaná disertační práce se zabývá lékařsko-geografickými aspekty pandemie HIV/AIDS. Práce si klade následující cíle: za prvné chce představit lékařskou geografii jakožto mezioborovou disciplánu, diskutovat její historický vývoj a naznačit její úlohu v současnosti a budoucnosti. Dalším cílem je analyzovat prostorové atributy celosvětové pandemie HIV/AIDS na dvou případových studiích – Ugandě a Ukrajině, a následně se pokusit provést srovnání regionálních specifik. Teoretický úvod vedle rozboru základních biomedicínských informací o infekci HIV/AIDS dikskutuje na základě literatury potenciální prostorové vazby ovlivňující zdravotní stav člověka v souvislosti s nákazou HIV a onemocněním AIDS. Rozbor využívá trojúhelníkové schéma humánní ekologie, které je aplikováno na první případové studii (Ugandu). V diskusi jsou zohledněny specifické faktory, které měly v minulosti vliv na šíření infekce v této zemi, a diskutovány úspěšné nástroje, díky nimž se daří epidemii v Ugandě významně eliminovat. Další část práce je zaměřena na Ukrajinu, vybranou jako druhé modelové území. Na základě korelační analýzy epidemiologických dat o prevalenci HIV/AIDS a kvantitativních socioekonomických dat byly hledány vazby mezi nárůstem prevalence HIV a vybranými faktory vyjadřující transformaci ukrajinské společnosti na cestě od socialismu k tržní ekonomice. Práce postihuje časoprostorové aspekty šíření infekce HIV v jednotlivých oblastech Ukrajiny. Na základě takto získaných poznatků jsou pak vymezeny specifické faktory, které v současnosti mohou na Ukrajině ovlivňovat šíření infekce, a po vzoru předchozí části jsou navrženy nástroje pro eliminaci této epidemie. Je potvrzen předpoklad, že na Ukrajině je infekce HIV především urbánním fenoménem, který příliš nesouvisí s chudobou a nízkou životní úrovní obyvatelstva. Rovněž rozkrývá fakt, že nárůst prevalence HIV souvisí s faktory porušené společenské soudržnosti. Tato práce podrobně zpracovává specifikami regionálních rozdílů faktorů, které ovlivňují epidemie HIV/AIDS. Je jednou z prvních takto zaměřených česky psaných prací a může přispět k renesanci lékařské geografie v České republice nebo na Slovensku.

Abstract:

This dissertation deals with the attributes of HIV/AIDS pandemics from the medical geography’s point of view. It includes the following: 1.) medical geography as a multidisciplinary domain is presented and discussed, with a focus upon its role both currently and then potentially for the future, 2.) an analysis of spatial attributes of the global HIV/AIDS pandemic, which is researched and evaluated through a case study of Uganda, 3.) HIV/AIDS epidemics in the Ukraine are geographically analyzed, and then, 4.) regional specifics of both Ugandan and Ukrainian epidemics are compared. Besides basic biomedical information regarding HIV infection, the theoretical outset of the work discusses all the possible links that can influence the health status of a human related to HIV/AIDS. This analysis is based on the triangle of human ecology. That triangle scheme is then applied to the first case country of Uganda. Various specific factors that influenced a diffusion of the virus within this country are taken into consideration based on scientific literature as well as field experience. Then, tools that have been proved as successful ones to significantly reduce the epidemics are specified. The next part is focused on the second case country of the Ukraine. Specific links between HIV prevalence growth and various factors implying the process of transformation from socialism to market economy have been researched based on correlation analysis of epidemiological and socioeconomic data sets. Time-space aspects of diffusion within Ukrainian regions are described and specific factors fueling the epidemics in this country are identified. This research has confirmed the hypothesis that HIV/AIDS in the Ukraine is specifically an urban phenomenon, and that it has low correlation with poverty or low living standards. There is another aspect which is quite obvious: that HIV prevalence increase strongly associates with factors of disrupted social cohesion. This work deals with the regional differences among factors that fuel or inhibits the epidemics of HIV. It is one of the very first publications written in Czech, and it can contribute to a “renaissance” of medical geography in the Czech Republic and perhaps also in Slovakia.
