Isolation and typing of Klebsiella spp. resistant to critically important antibiotics in the environment

Abstract:

Antibiotická rezistence dnes představuje velký problém pro veřejné zdraví, jelikož jsou rezistentní bakterie nalézány i v životním prostředí, oprávněně roste obava, že se lidstvo ocitne v situaci před objevem antibiotik. Odpadní vody řadíme mezi hlavní článek procesu šíření antibiotické rezistence. Cílem této diplomové práce bylo izolovat a charakterizovat soubor 365 Klebsiella spp. izolovaných z hospitalizovaných pacientů, nemocničních a městských odpadních vod, a přilehlých řek na území České republiky. Izoláty byly selektivně izolovány (cefotaxim nebo meropenem) a druhově určeny. U izolátů (n=122) získaných na půdě s meropenemem nebyl detekován gen rezistence ke karbapenemům, proto nebyly dále charakterizovány. U izolátů získaných na půde s cefotaximem (n=243) byl stanoven fenotyp rezistence (produkce beta-laktamáz a citlivost ke 24 antibiotikům) a bylo provedeno celogenomové sekvenování pro určení sekvenčních typů, genů rezistence a plazmidových replikonů. Izoláty Klebsiella spp. získané na půdě s cefotaximem byly původem ze všech lokalit kromě řeky. 88,47 % izolátů (n=243) produkovalo širokospektrou beta-laktamázu (ESBL) a 6,58 % AmpC beta-laktamázu. Všechny izoláty vykazovaly multirezistentní fenotyp, rezistence ke karbapenemům byla zaznamenána u 2 % izolátů k meropenemu a u 12 % k ertapenemu. Většina izolátů nesla gen kódující produkci ESBL blaCTX-M-15 (97,53 %), dále geny pro produkci úzkospektrých beta-laktamáz blaOXA-1 (81,48 %) a blaTEM-1 (76,54 %). Zároveň většina izolátů nesla geny rezistence k chinolonům (100 %), aminoglykosidům (96,70 %) a trimethoprimu (96,70 %). Byla detekována vysoká variabilita kmenů reprezentována 59 různými sekvenčními typy. Nejčastěji byly detekovány ST307 (13,58 %), ST29 (6,99 %) a ST5889 (4,93 %). Nejvíce se vyskytujícím plazmidovým replikonem byl IncFIB(K) u 76,54 % izolátů. V rámci této diplomové práce byl charakterizován rozsáhlý soubor multirezistentních izolátů Klebsiella spp. produkující zejména ESBL CTX-M-15. Bylo detekováno i 17 kmenů (6,99 %) produkující karbapenemázy GES-5, NDM-1, KPC-3 a OXA-48. Šest z nich bylo izolováno z odtoku čistírny odpadních vod. Získané výsledky potvrzují nedostatečný proces čištění odpadních vod a následné riziko kontaminace životního prostředí multirezistentními kmeny a ohrožení veřejného zdraví.

Abstract:

Nowadays, antibiotic resistance represents a major issue for public health, due to resistant bacteria being found in the environment, there is a legitimate fear that humanity will find itself in a situation similar to before the discovery of antibiotics. Wastewaters represents one of the main hotspots in the spread of antibiotic resistance. The aim of this thesis was to isolate and characterize a set of 365 Klebsiella spp. strains isolated from hospitalized patients, hospital wastewaters, municipal wastewater treatment plants (WWTPs), and adjacent rivers in the Czech Republic. The isolates were selectively isolated (cefotaxime or meropenem) and species identified. No carbapenem resistance gene were detected in the set of isolates (n=122) obtained on medium with meropenem, therefore they were not further characterized. In the isolates obtained on medium with cefotaxime (n=243), the resistance phenotype was determined (beta-lactamase production and susceptibility to 24 antibiotics) and whole-genome sequencing was performed to determine sequence types, resistence genes, and plasmid replicons of isolates. Klebsiella spp. isolates, obtained by cultivation with cefotaxime, originated from all sites except the river. 88.47 % of isolates (n=243) were producers of extended-spectrum beta-lactamase (ESBL) and 6.58 % AmpC beta-lactamase. All isolates showed a multi-resistant phenotype, resistance to carbapenems was detected in 2 % of isolates to meropenem and in 12 % to ertapenem. The majority of isolates carried a gene blaCTX-M-15 (97.53 %) encoding the production of ESBL, as well as the genes for the production of narrow-spectrum beta-lactamases blaOXA-1 (81.48 %) and blaTEM-1 (76.54 %). At the same time, most isolates carried resistance genes to quinolones (100 %), aminoglycosides (96.70 %), and trimethoprim (96.70 %). High diversity among strains was represented by 59 different sequence types. ST307 (13.58 %), ST29 (6.99 %), and ST5889 (4.93 %) were detected most frequently. The most abundant plasmid replicon was IncFIB(K) found in 76.54 % of the isolates. As part of this thesis, an extensive set of multi-resistant Klebsiella spp. isolates producing mainly ESBL CTX-M-15 were characterized. 17 strains (6.99 %) belonged to carbapenemases GES-5, NDM-1, KPC-3 and OXA-48 producers. Six of them were isolated from the WWTP effluent. The obtained results confirm the insufficient wastewater treatment process and the subsequent risk of environmental contamination by multi-resistant strains and the threat to public health.
