Nursing Process in Care of Patient after Cardiac Surgery Operation with Dehiscence Wound

Abstract:

Bakalářská práce je zaměřená na pacientku po kardiochirurgické operaci s dehiscentní ránou. Cílem bakalářské práce je vytvoření názorného plánu ošetřovatelské péče vycházejícího z ošetřovatelských diagnóz NANDA-I taxonomie II, který se vztahuje ke konkrétním potřebám jedince. Práce je rozdělená na teoretickou a praktickou část. Teoretická část obsahuje dvě základní kapitoly, vztahující se k aortokoronárnímu bypassu a chronické ráně. Je zde vysvětlen princip aortokoronárního bypassu, jeho nejčastější příčiny a perioperační péče. U chronické rány se zabýváme fázemi hojení, příčinami vzniku a léčbou rány pomocí negative pressure wound therapy. Praktická část je věnována ošetřovatelskému procesu u vybrané pacientky s dehiscentní ránou po aortokoronárním bypassu. Na základě získaných informací byly sestaveny ošetřovatelské diagnózy, k nimž byly vymezeny cíle ošetřovatelské péče, popsány intervence, realizace a následné vyhodnocení.

Abstract:

This bachelor´s thesis is focused on a patient after cardiothoracic surgery with dehiscence wound. The main aim of the thesis is preparation of a demonstrative plan of nursing care based on nursing diagnoses NANDA-I taxonomy II, which relates to one´s specific needs. The thesis is divided to theoretic and practical part. The theoretic part consists of two basic chapters related to the chronic wound. There is explained the principle of aortocoronary bypass, its most common causes and perioperative care. In the chronic wound part we deal with the phases of healing, causes of formation and treatment of the wound with the negative pressure wound therapy. Practical part is devoted to nursing process of a specific patient with dehiscence wound after aortocoronary bypass. Based on the information obtained the nursing diagnoses were formed to which the nursing aims were defined, further it contains description of interventions, realizations and following evaluation.
