Abstract:

Úvod: V současné době je jedinou akceptovanou diagnostickou metodou svalovinu infiltrujícího karcinomu močového měchýře histologické vyšetření získané transuretrální resekcí tumoru. Rozlišení svalovinu infiltrujícího a neinfiltrujícího tumoru je zásadní nejen z hlediska prognózy pacienta, ale především s ohledem na naprosto odlišný způsob léčby. Zatímco u neinfiltrujícího karcinomu močového měchýře je endoskopická resekce tumoru dostatečnou, a v mnoha případech definitivní léčbou, v případě svalovinu infiltrujícího karcinomu je nutno provést radikální cystektomii. Magnetická rezonance zažívá na poli urologie v posledním desetiletí velký rozmach a její využití v diagnostice karcinomu prostaty bylo doslova průlomové. Analogický systém multiparametrické magnetické rezonance (Vesical Imaging-Reporting and Data System - VI-RADS) si v diagnostice karcinomu močového měchýře dává za cíl odlišit svalovinu infiltrující a neinfiltrující karcinom bez nutnosti histologického vyšetření, ale s vysokou mírou specificity a senzitivity. To by pro řadu pacientů znamenalo možnost ušetřit je operačního výkonu nezbytného k určení hloubky invaze do svaloviny a v indikovaných případech tak urychlit jejich léčbu. Cíl: Cílem práce bylo zhodnotit výtěžnost systému VI-RADS v rozlišení svalovinu infiltrujícího karcinomu močového měchýře od neinfiltrujícího a určit odpovídající hraniční (cut-off) hodnotu. Zaměřili jsme se ale také na přínos VIRADS v každodenní klinické praxi z pohledu urologa. Materiál a metoda: Od března 2020 do prosince 2021 bylo do souboru zařazeno celkem 64 pacientů s tumorem močového měchýře diagnostikovaným cystoskopicky, ultrazvukem či počítačovou tomografií, indikovaných k transuretrální resekci tumoru. Medián věku pacientů byl 68 let. Před výkonem byla provedena multiparametrická magnetická rezonance a nález byl popsán systémem VI-RADS spolupracující radioložkou. Následně byla u všech pacientů provedena transuretrální resekce tumoru. Na základě porovnání výsledku histologického vyšetření a VI-RADS skóre byla stanovena senzitivita, specificita, pozitivní prediktivní hodnota a negativní prediktivní hodnota. Výtěžnost metody byla hodnocena jak pro cut-off hodnotu VI-RADS ? 3, tak VI-RADS ? 4. Výsledky: V našem souboru byla pro cut-off VI-RADS ? 3 senzitivita 92,3 %, specificita 81,4 %, pozitivní prediktivní hodnota 52,2 % a negativní prediktivní hodnota 98,0 %, respektive 76,9 %, 91,5 %, 66,7 % a 94,7 % pro VI-RADS ? 4. S ohledem k nejvyšší celkové přesnosti 83,3 % bylo skóre VI-RADS ? 3 stanoveno jako určující pro infiltraci svaloviny. Nicméně z hlediska absolutních čísel byla ve skupině VI-RADS ? 3 většina pacientů se svalovinu neinfiltrujícím karcinomem močového měchýře. Závěr: Studie potvrdila vysokou senzitivitu i specificitu systému VI-RADS odpovídající sledovaným literárním zdrojům. Vzhledem k absolutním číslům a zkušenosti s využitím tohoto systému v praxi, nejsme ale toho času přesvědčeni o jeho přínosu natolik, abychom považovali multiparametrickou magnetickou rezonanci za univerzálně použitelnou alternativu transuretrální resekce tumoru močového měchýře.

Abstract:

Introduction: Currently, the only accepted method how to exclude muscle-invasive bladder cancer is the histological examination obtained from transurethral resection of the tumour. The differentiation between a muscle-invasive and a non-muscle invasive tumour is essential. It is very important because of the patient's prognosis and also with regard to the completely different methods of treatment. While endoscopic resection of the tumour is sufficient treatment for non-muscle invasive bladder cancer, radical cystectomy must be performed in the case of muscle-invasive cancer. Magnetic resonance imaging has been experiencing a great boom in the urology field in recent decades, and its use in the diagnosis of prostate cancer was literally ground-breaking. Analogous system of multiparametric magnetic resonance (Vesical Imaging-Reporting and Data System - VI-RADS) in the diagnosis of bladder cancer aims to differentiate between muscle-invasive and non-muscle invasive cancer without the need for histological examination, but with a high degree of specificity and sensitivity. This would mean the possibility of saving many patients from the surgical procedure necessary to obtain histology and determine the depth of invasion into the muscle tissue and, in indicated cases, to speed up their treatment. Aims: The aim of the work was to evaluate the yield of the VI-RADS system in differentiating muscle-invasive from non-muscle invasive bladder cancer and to determine the cut-off value of VI-RADS. However, we also focused on the contribution of this system in everyday clinical practice from the point of view of a urologist. Methods: 64 patients with bladder cancer were enrolled into this prospective study between March 2020 and December 2021. Median age of patients was 68 years. Multiparametric magnetic resonance imaging was performed before transurethral resection of the bladder tumour and evaluated using the VI-RADS score. Score were compared to histopathology results. We evaluated the sensitivity, specificity, positive and negative predictive value of this system using both cut-off VI-RADS ? 3 and ? 4. Results: Sensitivity of 92,3 %, specificity of 81,4 %, positive predictive value of 52,2 % and negative predictive value of 98,0 % was determined using cut off VI-RADS ? 3, while sensitivity of 76,9 %, specificity of 91,5 %, positive predictive value of 66,7 %, and negative predictive value of 94,7 % was determined using cut-off VI-RADS ? 4. Based on our results, we consider the optimal cut-off point to be VI-RADS ? 3 with the overall prediction accuracy of 83,3 %. However, in terms of absolute numbers, the majority of patients in VI-RADS ? 3 group were the patients with non-muscle invasive bladder cancer. Conclusion: We acknowledge that multiparametric magnetic resonance provides valuable information with regard to bladder cancer staging, however, despite its high overall accuracy, currently we do not consider the VI-RADS could replace transurethral resection of tumour in discrimination between non-muscle invasive and muscle-invasive bladder cancer.
