Rehabilitation after injury of the tendon system of the hand

Abstract:

Úvod: Tématem této bakalářské práce je rehabilitace po poranění šlachového aparátu ruky. Práce se zaměřuje na poranění šlachového poutka, což je v současné době jedno z nejčastějších lezeckých poranění. Teoretická část se nejprve zabývá anatomií a kineziologií ruky jako takové, klasifikuje poranění šlachového aparátu, a poté zpracovává dosavadní poznatky o šlachových poutkách a mechanismech jejich poranění. V práci jsou zmíněny možnosti jejich prevence, diagnostiky a následné terapie. Speciální část zahrnuje kazuistiku pacientky s poraněním šlachového poutka a popisuje fyzioterapeutické postupy u tohoto typu poranění. Cíl práce: Cílem bakalářské práce je přispět k řešení problematiky fyzioterapie u pacienta s poraněním šlachového aparátu ruky. Metodika: V této bakalářské práci je zpracována kazuistika dvacetileté ženy po ruptuře šlachového poutka. Kapitola obsahuje vstupní kineziologické vyšetření, které zahrnuje odebrání anamnézy, antropometrii, goniometrii, statické a dynamické vyšetření pohybového aparátu a další. Na základě výsledků ze vstupního vyšetření byl stanoven krátkodobý fyzioterapeutický plán s využitím metod a konceptů fyzioterapie (TMT, MOB, pasivní a aktivní pohyb, PIR, PFS, DNS, ACT, PNF, Brügger koncept, cvičení s pomůckami). Celá spolupráce s pacientkou trvala 4 týdny a zahrnovala 8 terapií. Terapie probíhaly 2x týdně po dobu jedné hodiny. První čtyři terapie probíhaly na ambulantním oddělení a další čtyři v domácím prostředí a v prostorách lezecké stěny. V průběhu fyzioterapeutické intervence proběhlo kontrolní vyšetření. Po skončení krátkodobého fyzioterapeutického plánu byl proveden výstupní kineziologický rozbor. Výsledky: Z výstupního kineziologického vyšetření vyplývá, že došlo ke snížení otoku, zvětšení rozsahu pohybu a zvýšení svalové síly poraněného prstu. Závěry: Cíle a úkoly práce byly splněny.

Abstract:

Introduction: The topic of this bachelor thesis is rehabilitation after hand tendon injury. The thesis is focused on tendon pulley injuries, which is currently one of the most common climbing injuries. Firstly, the theoretical part is focused on anatomy and kinesiology of the hand, classifies tendon injuries and then points out the current knowledge about pulley injuries and describes the mechanism of the injury. The possibilities of the prevention, diagnosis and following therapy are discussed. The special part includes a case report of a patient with tendon pulley injury and describes physiotherapeutic methods for this type of injury. The objective: The aim of this thesis is to contribute to the existing solution of choosing the most suitable physiotherapeutic methods in a patient diagnosed with a tendon injury of the hand. Methodology: This bachelor thesis deals with a case report of a twenty-year-old woman after tendon pulley rupture. This part includes an initial kinesiological analysis, which includes anamnesis, anthropometry, goniometry, static and dynamic examination of the musculoskeletal system and more. Based on the results from the initial analysis, a short-term physiotherapy plan was determined using physiotherapy methods and concepts (TMT, MOB, passive and active movement, PIR, PFS, DNS, ACT, PNF, Brügger concept, exercises with aids). The whole cooperation with the patient lasted 4 weeks and included 8 therapies. The therapies were twice a week for one hour. The first four therapies took place at the ambulance and the next four at home and at the climbing wall. A follow-up examination was performed during the physiotherapy intervention. At the end of the short-term physiotherapy plan, an exit kinesiological analysis was performed. Results: The outcome of the kinesiological examination shows that the swelling was reduced and the range of motion and the muscle strength of the injured finger increased. Conclusions: The goals and the aims of the thesis were accomplished.
