Optimalization of diagnostics and therapy in adult patients with acute myeloid leukemia younger than 60 years

Abstract:

Úvod. Akutní myeloidní leukémie je závažné maligní onemocnění krvetvorby, které vzácně postihuje nemocné mladší 60 let věku. Centralizace diagnostiky a léčby těchto nemocných do velkých center umožňuje použití vysoce sofistikovaných metod laboratorní diagnostiky včetně metod molekulární biologie, podání individualizované inovativní terapie a řešení těžkých komplikací podané léčby. Prognóza AML však zůstává i nadále nepříznivá, což souvisí především se značnou heterogenitou onemocnění na molekulární úrovni. Poznání molekulárně genetického pozadí AML a oprava poškozených signálních drah leukemických buněk se ukazuje býti správným krokem k individuálnímu léčebnému přístupu a zlepšení prognózy nemocných. Umožňuje podání cílené léčby, sledování minimální zbytkové nemoci (MRD) a intenzifikaci léčby v případě vysokého rizika relapsu. Důležitým předpokladem dobrých výsledků léčby mladších nemocných s AML je rovněž informovanost pacienta a spolupráce mezi pacientem a ošetřujícím lékařem. Metody. Disertační práci tvoří obecný úvod a dále pak komentovaný soubor publikovaných prací týkajících se moderní diagnostiky a léčby AML a poznatků směřujících k její optimalizaci. Výsledky. Společně s kolegy ze skupiny CELL jsme aktualizovali databázi DATOOL-AML, která se stala zdrojem celonárodních „real-world“ dat pro analýzu diagnostického a terapeutického přístupu k dospělým nemocným s AML v České republice. Prokázali jsme, že výsledky centralizované intenzivní léčby AML v ČR korespondují s výsledky, které jsou dosahovány v mezinárodních klinických studiích a na renomovaných zahraničních pracovištích. V rámci laboratorní diagnostiky jsme pomocí NGS vyšetření na velkém souboru pacientů s novou AML popsali nejčastěji mutované geny a podařilo se nám definovat několik nových prognostických skupin, které profitují z provedení alogenní transplantace krvetvorby v první kompletní remisi. Studovali jsme mutace genů z pohledu jejich klonálního vývoje, a to rovněž na úspěšně vytvořeném myším modelu. Dále jsme objevili dosud nepopsanou mutaci v genu NPM1, což umožnilo sledovat individuální MRD u našeho pacienta. V rámci mezinárodního projektu jsme popsali frekvenci chromotripse u nemocných s AML a prokázali jsme její asociaci s vysoce rizikovými cytogenetickými abnormalitami a nepříznivou prognózou AML. Vydáním informační brožury pro pacienty jsme přispěli k lepší informovanosti pacientů, která je předpokladem dobré spolupráce a adherence k léčbě. Závěr. Zpracovaná témata a výsledky disertační práce přispěly k optimalizaci léčby dospělých nemocných s AML pod 60 let.

Abstract:

Background. Acute myeloid leukemia (AML) is a malignant hematopoietic disorder that is rarely diagnosed in patients under the age of 60. The centralization of diagnostic and treatment procedures into high volume centres allows the use of highly sophisticated laboratory methods, including methods of molecular biology, the administration of an innovative therapy and the effective management of severe complications after the intensive therapy. Nevertheless, the prognosis of AML remains unfavourable that is dominantly related to the heterogeneity of the disease at the molecular level. The research of genetic and molecular background of AML and the recovery of the affected signaling pathways in leukemic cells seems to be the key for improving the prognosis of patients. This research allows the administration of a targeted treatment, the monitoring of the minimal residual disease (MRD) and the intensification of treatment in case of high-risk patients. The awareness and good compliance of patients is another key factor for a successful treatment of younger patients. Methods. This thesis covers a general overview and a collection of published papers regarding modern AML therapy, and points out to its further optimization. Results. Together with colleagues from the CELL group, we updated the DATOOL-AML database, which has become a source of nationwide real-world data for the analysis of diagnostic and therapeutic approaches to younger adult patients with AML in the Czech Republic. We have shown that the results of centralized intensive care for AML in the Czech Republic correspond to the results that were achieved in international clinical trials and in foreign hospitals. As part of laboratory diagnostics, we described the most frequently mutated genes using the next generation sequencing in a large group of patients with de novo AML. We were also able to define several new prognostic groups that benefit from performing allogeneic HSCT in the first complete remission. We studied the clonal evolution of leukemic cells as well as in a successfully developed mouse model. Furthermore, we discovered an undescribed mutation in the NPM1 gene, which enabled the creation of an individual assay for monitoring MRD in the case of our patient. In an international project, we described the frequency of chromothripsis in patients with AML and demonstrated its association with high-risk cytogenetic abnormalities and an unfavourable prognosis of AML. By publishing an information brochure for patients, we have contributed to improve patient awareness, which is crucial for a good cooperation and adherence to the treatment. Conclusions. In summary, all the mentioned results of this thesis considerably contributed to optimize the care of acute myeloid leukemia in younger adults.
