Virulence characteristics of E. coli strains isolated from hemoculture

Abstract:

Escherichia coli is a commensal organism in human intestinal tract. Some strains are responsible for various extraintestinal diseases. E. coli is the most frequent cause of community-acquired sepsis. The main aim of the thesis was to determine virulence characteristics of E. coli strains isolated from blood of patients. The genetic profile of bacterial strains consists of phylogenetic group, identification of 18 virulence characteristics, identification of bacteriocins and detection of genotoxin colibactin. A set of 314 Escherichia coli strains were examined. Detection of the genes was performed by PCR followed by gel electrophoresis. Studied E. coli strains belonged mainly to phylogenetic groups B2 (58.9%) and D (22.0%), less frequently to groups A (14.3%) and B1 (7.6%). The most frequent virulence factors were adhesins (fimA 85,7%, pap 47.1%, sfa 26.8%), followed by aerobactin genes (aer, 70.7%, iucC, 68.2%) and cytotoxic necrotizing factor (cnf1, 11.1%). These genes were present in a higher frequency in phylogroup B2. Considering the bacteriocins, the identification showed higher frequency of microcins in strains from hemoculture in comparison to commensal intestinal strains. This was caused mainly by high presence of microcin M (41.1%; p < 0.01). The obtained data, especially high frequency of aerobactin genes in combination with the microcin M (both asociated with iron acquisition) suggest that these genes encode virulence factors important for invasion and survival in the bloodstream.

Abstrakt Escherichia coli je komenzálním organismem v trávicím traktu lidí. Také však může způsobovat různé extraintestinální infekce a je nejčastějším původcem komunitně získaných sepsí. Cílem práce bylo stanovit virulentní vlastnosti kmenů E. coli izolovaných z krve pacientů. Genetický profil bakteriálních kmenů spočíval v zařazení do fylogenetických skupin, identifikace 18 genů kódujících virulentní faktory, určení frekvence bakteriocinogenie, identifikace jednotlivých bakteriocinových typů a detekce genotoxinu kolibaktinu. Vyšetřovaný byl soubor 318 kmenů Escherichia coli. Použitou metodou byla PCR detekce jednotlivých genů následována gelovou elektroforézou. Kmeny E. coli izolované z hemokultur patřily především do fylogenetických skupin B2 (58,9%) a D (22,0%), méně často do skupin A (14,3%) a B1 (7,6%). Nejfrekventovanějšími virulentními faktory byly adheziny (fimA - fimbrie typu I, 85,7%; pap - P-fimbrie, 47,1%; sfa – S-fimbrie, 26,8%), následovány geny kódujíci aerobaktin (aer, 70,7%; iucC 68,2%) a cytotoxický nekrotizující faktor (cnf1, 11,1%). Tyto geny zároveň patřily častěji do fylogenetické skupiny B2. Byla zjištěna vyšší produkce mikrocinů ve srovnání s komenzálními střevními kmeny, především prevalence mikrocinu M dosáhla 41,1% (p <0.01). Získané výsledky naznačují, že kmeny E. coli patřící do fylogenetické skupiny B2 a D, kódující adheziny, systémy na získávání železa, toxiny a mikrociny, především mikrocin M, jsou nejčastěji detekovanými patogeny izolovanými z lidského krevního řečiště.
