Complications associated with chorioamnionitis

Abstract:

Úvod: Tato diplomová práce se zabývá problematikou chorioamnionitidy a intraamniálních infekcí s identifikací dopadu těchto těhotenských komplikací na perinatologické výsledky. V teoretické části předkládá obecné poznatky, ve které je v širších souvislostech popsána definice onemocnění, etiopatogeneze, prevalence, diagnostika a managment terapie na základě nejnovější poznatků. Zásadní část práce pojednává o možných mateřských komplikacích a komplikací, které ohrožují zdraví a život novorozence, zejména pak v případě předčasného porodu. Metody: Výzkumná část práce je vedena retrospektivní analýzou dat získaných z dokumentace pacientek, které byly hospitalizované v letech 2017-2024 ve FN Brno s diagnózou chorioamnionitis, potvrzené na základě zvýšené hladiny interleukinu-6 (IL-6) v plodové vodě. Jde o kvantitativní typ výzkumu, čítající výzkumný vzorek 37 žen a 37 novorozenců. U žen probíhala analýza dat s ohledem na gestační týden, vývoj zánětlivých markerů, způsobu vedené terapie, délku latentní doby (oddálení porodu) a výskyt mateřských komplikací. U novorozenců pak výzkumné šetření cílilo na morbiditu a mortalitu. Výsledky: Bylo stanoveno a následně ověřováno 11 hypotéz. Výsledkem statistického zpracování získaných dat bylo potvrzeno, že chorioamnionitida, respektive intraamniální infekce, je statisticky významným faktorem ovlivňujícím perinatologické výsledky. Největší statistická významnost byla zjištěna u závažných novorozeneckých komplikací. Potvrzení stanovených hypotéz této práce podtrhuje důležitost včasné diagnostiky a cílené léčby.

Abstract:

Introduction: This thesis addresses the issue of chorioamnionitis and intra-amniotic infections with identification of the impact of these pregnancy complications on perinatal outcomes. The theoretical part presents the general knowledge in which the definition of the disease, etiopathogenesis, prevalence, diagnosis and management of therapy are described in a broader context based on the latest evidence. The essential part of the thesis discusses the possible maternal complications and complications that threaten the health and life of the newborn, especially in the case of preterm birth. Methods: The research part of the thesis is guided by a retrospective analysis of data obtained from the records of patients who were hospitalized in 2017-2024 at the Brno University Hospital with a diagnosis of chorioamnionitis, confirmed by elevated levels of interleukin-6 (IL-6) in amniotic fluid. This is a quantitative type of research composed of 37 women and 37 newborns. The women were analyzed for data with respect to gestational age, development of inflammatory markers, type of therapy administered, length of latent phase (delayed delivery), and incidence of maternal complications. In neonates, the research was aimed at morbidity and mortality. Results: Eleven hypotheses were established and subsequently tested. Statistical processing of the data confirmed that chorioamnionitis, or intra-amniotic infection, is a statistically significant factor affecting perinatal outcomes. The highest statistical significance was found for major neonatal complications. The confirmation of the established hypotheses of this study underlines the importance of early diagnosis and targeted treatment.
