Detection of antibodies against interferon beta in monitoring of treatment of patients with multiple sclerosis

Abstract:

Interferon beta patří mezi léky první volby v léčbě relaps remitentní formy roztroušené sklerózy. Jedním z nežádoucích účinků tohoto léku je však tvorba protilátek proti interferonu beta, tyto protilátky se nazývají vazebné. Podskupinou vazebných protilátek jsou neutralizační protilátky, které mají schopnost snižovat biologickou aktivitu interferonu a tím i negativně ovlivnit účinek léku. Současné metody pro měření neutralizačních protilátek jsou poměrně nákladné, pracné a časově náročné, tudíž byla v bakalářské práci prováděna detekce vazebných protilátek proti třem různým preparátům (Avonexu, Rebifu a Betaferonu), a to za pomocí metody ELISA. Pro hodnocení bioaktivity interferonu beta lze využít i stanovení biomarkerů léčby, mezi které patří například protein MxA nebo chemokin CXCL10. Stanovení těchto dvou biomarkerů metodou ELISA bylo rovněž součástí této práce. Následně byly zjišťovány korelace mezi jednotlivými analytickými parametry a klinickými daty. Analýzou byly nalezeny odlišnosti v tvorbě vazebných protilátek mezi pacienty užívající odlišné preparáty interferonu beta, kdy největší hodnoty a také nejvíce pozitivních vzorků bylo naměřeno u pacientů léčených interferonem ?-1b. Nejvýznamnější korelace byla zjištěna u koncentrací vazebných protilátek proti jednotlivým interferonům použitým pro potah destičky.

Abstract:

Interferon beta belongs to the group of first-choice drugs for the treatment of relapsing-remitting multiple sclerosis. However, one of the side effects of this drug is production of antibodies against interferon beta called binding antibodies. A subset of binding antibodies are neutralizing antibodies, that are able to decrease interferon bioavailability and thereby negatively affect the effect of the drug. The current methods for measuring neutralizing antibodies are rather expensive, technically difficult and time-consuming, therefore the method chosen for bachelor thesis was detection of binding antibodies against three types of interferon beta preparations (Avonex, Rebif and Betaferon) using ELISA assay. Bioactivity of interferon beta can be also assessed using assays for determination of biomarkers, e.g. determination of MxA protein or chemokine CXCL10. Measuring of these two biomarkers were also a part of the thesis. The results were compared with clinical data and the correlations between these stastistical data were investigated. Differences were found in the production of binding antibodies between patients receiving different interferon beta preparations. The highest values as well as the most positive samples were measured in patients treated with interferon beta-1b. The most significant correlation was found in the concentration of binding antibodies to individual interferons used for coating microtiter plate.
