Use of magnetic field sensors for distal locking of intramedullary nails in long bone fractures

Abstract:

Intramedulární hřebování je preferovanou metodou osteosyntézy u diafyzárních zlomenin dlouhých kostí. Distální zajištění, které je součástí hřebové osteosyntézy je běžně prováděno pod kontrolou rentgenového zesilovače. Cílem práce bylo prověřit vlastnosti a funkčnost nového navigačního zařízení pro distální jištění intramedulárních hřebů bez nutnosti rentgenové kontroly. Práce podává základní charakteristiku navigační sondy, jednotlivých komponent a principů, na nichž je založen navigační algoritmus. Na základě výzkumu v laboratorních podmínkách byla zhodnocena přesnost nově navržené navigace, jež nevyžaduje rentgenovou kontrolu, a možná úskalí navigovaného distálního jištění. Výsledky práce prokazují potenciál navigačního zařízení detekovat distální otvory hřebu a následné zavedení jistícího šroubu bez nutnosti radiační zátěže.

Abstract:

Intramedullary nailing is the preferred method of osteosynthesis for diaphyseal fractures of long bones. Distal locking, a tricky part of nail osteosynthesis, is commonly performed under the control of an X-ray intensifier. The aim of this dissertation was to investigate the properties and functionality of a new navigation device for distal locking of intramedullary nails without the need for X-ray control. The thesis presents the basic characteristics of the navigation probe, individual components, and principles on which the navigation algorithm is based. Based on research in laboratory conditions, the accuracy of navigation without radiation exposure and possible pitfalls of navigated distal locking were evaluated. The results of our research point to the device's ability to detect the distal hole of the nail and the subsequent insertion of the locking screw without radiation exposure.
