Nursing Process in Care of Patient with Tracheostomy

Abstract:

Bakalářská práce se věnuje problematice ošetřovatelské péče o pacienta s tracheostomií. V teoretické části, která je rozdělena do dvou kapitol, je přiblížena role zdravotní sestry od indikace tracheostomie u pacienta, přes následnou péči až k dekanylaci. Praktická část se zabývá názorným přiblížením klinické praxe, kdy cílem bakalářské práce bylo vytvoření ošetřovatelského procesu u konkrétního pacienta s tracheostomií na oddělení intenzivní péče, který vychází z ošetřovatelských diagnóz stanovených dle NANDA International Taxonomie II. Ošetřovatelský proces je členěn do pěti fází, které byly uplatněny i při aplikaci jednotlivých ošetřovatelských intervencí. Při tvorbě ošetřovatelského plánu pro pacienta jsme diagnostikovali celkem 19 diagnóz, z toho 7 diagnóz patří do vnímaného rizika. Na podkladě 6 prioritních ošetřovatelských diagnóz byly vytyčeny ošetřovatelské cíle. Pro chronické progresivní onemocnění vybraného pacienta jsme se soustředili na cíle dlouhodobé. Díky těmto cílům, jsme naplánovali a realizovali intervence založené na individuálních potřebách pacienta. V závěrečném hodnocení jsme došli ke splnění 4 ošetřovatelských cílů a 2 cílů splněných částečně ve 3 dnech realizace ošetřovatelské péče.

Abstract:

The bachelor thesis is focused on a nursing care for patients with tracheostomy. The theoretical part, which is divided into two chapters, describes the role of the nurse from the indication of tracheostomy in the patient, through subsequent care to decanylation. The practical part deals with a clear approach to clinical practice, where the aim of the bachelor thesis was to create a nursing process for a specific patient with tracheostomy in the intensive care unit, which is based on nursing diagnoses determined by NANDA International Taxonomy II. The nursing process is divided into five phases, which were also applied in the application of individual nursing interventions. When creating a nursing plan for a patient, we diagnosed a total of 19 diagnoses, of which 7 diagnoses belong to the perceived risk. Nursing goals were set based on 6 priority nursing diagnoses. For chronic progressive disease of a selected patient, we focused on long-term goals. According to these goals, we planned and implemented interventions based on the individual needs of the patient. In the final evaluation, we accomplished 4 nursing goals and 2 goals were partially fulfilled in 3 days of nursing care.
