Oral health state of 13-15 year-old children, the relationship between its indicators and genetic background

Abstract:

Řada literárních údajů nasvědčuje tomu, že mezi rizikové faktory vzniku zubního kazu a parodontopatií u dětí patří nejenom lokální noxy, ale také další faktory jako je zdravotní stav v průběhu dětství a vzdělání rodičů. Ukazuje se rovněž, že tyto choroby mohou mít i genetické pozadí. Cílem naší práce bylo posoudit v retrospektivní studii vztah mezi přítomností kazu a výskytem a stupněm gingivitidy u 13 – 15letých dětí sledovaných v rámci skupiny ELSPAC Brno a stupněm vzdělání jejich rodičů, závažností přítomných ortodontických anomálií, stupněm opacit skloviny, spektrem a množstvím parodontálních patogenů a zhodnotit vztah vybraných polymorfismů v genech pro rozvoj sledovaných chorob. Náhodně vybrané děti ze skupiny ELSPAC Brno (n=780), jejichž rodiče souhlasili se zařazením do studie, byly vyšetřeny na Stomatologické klinice LF MU a FN u sv. Anny v Brně. U dětí jsme zaznamenali KPE index zubů (dle kriterií WHO, D3MFT) i výskyt opacit skloviny (Dean´s fluorosis index), gingivitidu jsme hodnotili pomocí modifikovaného gingiválního indexu (GI) (Löe- Silness), přítomnost plaku a zubního kamene pomocí indexu PI a CSI. Přítomnost/absenci ortodontických anomálií jsme hodnotili pomocí indexu IOTN. Zaměřili jsme se i na sledování asociace mezi KPE/GI a úrovní vzdělání rodičů. Údaje o vzdělání rodičů (PEL) a podávání antibiotik jsme získali z databáze ELSPAC. Přítomnost/absenci parodontálních patogenů jsme sledovali v odběrech na papírových čípcích zavedených do sulkulárního žlábku, materiál byl hodnocen pomocí soupravy ParoCheck®.. V rámci genetického vyšetření byly ze získané sliny analyzovány polymorfismy v genech pro interleukin-18 (IL 18), interleukin -6 (IL-6), pro matrix metaloproteinázu 9 (MMP-9) a ACE I/D. Ke statistickému hodnocení získaných výsledků byly použity standardní parametrické i neparametrické testy (Statistica v. 8.0., Statsoft Inc., Tulsa, USA) a SPSS.10 (Base, Regression-multilevel regression model). Výsledky práce ukázaly, že index KPE souboru je relativně vysoký (KPE = 2,82), podíl dětí bez kazu činil 25,4%. Průměrná hodnota GI indexu kohorty byla 0,204, stupeň 0 byl nalezen u 36,9% dětí, stupeň 1 u 43,0% a stupeň 2 u 19,5%. Statisticky signifikantní asociace byla zjištěna mezi GI a KPE, GI a složkou P, GI a stupněm závažnosti ortodontické anomálie, dále mezi GI zubů intaktních a s výplní vs zubů s potřebou ošetření a mezi hodnotou PI u dětí s gingivitidou vs dětem zdravým (p<0.05). Úroveň vzdělání matky (nikoli však otce) signifikantně negativně korelovala s KPE indexem u dětí. Souvislost mezi vzděláním rodičů a manifestací gingivitidy a mezi podáváním ATB a opacitami skloviny se nám nepodařilo prokázat (p>0.05). Sledované parodontální patogeny byly přítomny běžně v dutině ústní u obou skupin (děti se zdravým parodontem, děti s gingivitidou), signifikantně vyšší počet byl detekován u dětí se zánětem gingivy (p<0.05). Děti s gingivitidou měly i signifikantně vyšší výskyt parodontálních patogenů u zubů bez zánětu než děti se zdravou gingivou (p<0.05). Analýza asociace haplotypů interleukinu-6 (IL-6) s plakem podmíněnou gingivitidou prokázala signifikantní rozdíly v jejich frekvenci mezi pacienty a zdravými jedinci (p<0.01). Výsledky nasvědčují tomu, že by se tento polymorfismus mohl uplatňovat v patogenezi zánětu dásní. IL-18-607C a MMP-9-1562T alely byly nalezeny u 58,9% a 8,3% zdravých dětí a u 62,2% a 15,0% pacientů s gingivitidou. Co se týče vztahu polymorfismu MMP-9 a IL 18 ke gingivitidě, ukázalo se, že zatímco rozdíly ve frekvenci alel u IL 18 mezi zdravými jedinci a pacienty s gingivitidou statisticky významné nebyly, MMP-9 polymorfismus vykazoval signifikantní rozdíly (p<0.05). významný vztah ke gingivitidě byl zjištěn u složeného genotypu obou variant genů (p<0.05). Interakci MMP-9 a IL 18 je tedy možno považovat za rizikový faktor pro vznik gingivitidy u dětí. Analýza ACE I/D polymorfizmu u dětí bez kazu a s kazem ukázala signifikantní rozdíl mezi dětmi s KPE 0 a KPE ≥4 (p<0.01). Je tedy možno jej považovat za rizikový faktor pro vznik zubního kazu ve stálé dentici. Práce ukázala relativně vysoký index KPE, nízký výskyt gingivitidy, významný vztah mezi GI a KPE, obzvláště u K složky indexu a dále mezi GI a ortodontickými anomáliemi. Potvrdila dále signifikantní vztah mezi stupněm vzdělání matek (nikoli otců) a kazivostí chrupu u dětí, zatímco vztah mezi vzděláním rodičů a stupněm gingivitidy u jejich dětí a vztah mezi podáváním ATB a opacitami skloviny se nepodařilo prokázat. U dětí s gingivitidou byl detekován vyšší počet parodontálních patogenů a to i u zubů bez zánětu. Výsledky práce nasvědčují tomu, že některé ze studovaných polymorfismů genů mohou mít vztah k rozvoji zubního kazu a/nebo gingivitidy.

Abstract:

A considerable body of evidence has shown that caries and gingivitis risk factors involve not only local noxious influences, but also another factors as the parental educational level (PEL) as well as health status in early childhood may play a significant role in dental caries/gingivitis development. Recently, genetic background was found to influence predisposition to dental caries and gingivitis manifestation in children. To assess in the retrospective study the relationship between caries experience and gingivitis manifestation in 13- to 15-yr-olds, involved into the multicentric European Longitudinal Study of Pregnancy and Childhood (ELSPAC) conducted also in Brno, and the PEL of their parents, orthodontic anomalies severity, enamel opacities presence and degree, spectrum and amount of periodontal pathogens and evaluate the relationship of selected gene polymorphisms to development of the given diseases. Randomly selected children from the ELSPAC cohort in Brno (n=780) were orally examined at the Clinic of Stomatology, FM MU and their dental status and gingivitis were recorded. The following clinical parameters were assessed: DMFT score and its components (D3MFT [WHO criteria]), the presence of enamel opacities (Dean´s fluorosis index), gingival index (GI), plaque index (PI), and calculus index (CSI). Gingivitis was measured using the modified Löe-Silness GI index. The presence of plaque and calculus was recorded according Silness-Löe (PI) and calculus surface index (CSI), respectively, without any disclosing agents. GI, PI and CSI were recorded on selected teeth. The presence/absence of orthodontic anomalies and their severity was recorded (IOTN index). The sulcular fluid samples taken on endodontic paper points were used for the identification of periodontal pathogens. Bacterial species were identified ParoCheck® kits. Within the scope of genetic investigations, saliva samples were analyzed for polymorfismus in the genes for interleukin-18 (IL 18), interleukin-6 (IL 6), matrix metalloproteinase 9 (MPP-9) and ACE I/D. Data on the PEL were retrieved from the ELSPAC database, as well as those on antibiotics administration. Parents of children signed the informed consent. Standard parametric and nonparametric tests were used to assess the results statistically. (Statistica v. 8.0., Statsoft Inc., Tulsa, USA) a SPSS.10 (Base, Regression-multilevel regression model). Mean DMFT of the group was 2.82 (SE 0.36), the share of caries free children was 25.4%. Mean GI index of the cohort was 0.204 (SE 0.011), grade 0 was found in 36.9% of children, grade 1 in 43.0%, and grade 2 in 19.5%. Statistical differences (p0.05), were found in GI vs DMFT, in GI of caries free and treated children vs treatment need, in GI vs D value, in GI vs severity of orthodontic anomaly and in value of PI between children with gingivitis vs healthy. Mothers’ (but not fathers’) educational level significantly negatively correlated with D3MFT scores of children (p<0.05). Similar correlation between GI and PEL was not found as well as between ATB administration and enamel opacities (p>0.05). The microbiological findings showed the significant correlation between the detection frequency of some periodontopatic bacteria and gingival pathology (p<0.025). Complex analysis of association of IL-6 haplotypes with plaque induced gingivitis revealed significant differences in haplotype frequencies between patients and healthy subjects (p<0.01). The results indicate that the three promoter polymorphisms in the IL-6 gene act in a corresponding cooperative fashion and suggest that IL-6 haplotypes could play a role in the pathogenesis of gingivitis. rmore, a highly significant association of the composite genotype (formed by the variant of both genes) with gingivitis was found (p<0.05). Analysis of ACE I/D polymorphism in children without and with caries showed significant difference between children with DMFT = 0 and DMFT ≥4 (p<0.01). The results demonstrated relatively high caries experience, low level of gingival inflammation and relation between GI and DMFT, particularly in D component, and between GI and orthodontic anomalies. The study confirmed a significant correlation of the educational level of mothers and caries experience in children (p<0.05), while the correlation between GI and PEL was not significant as well as between ATB administration and enamel opacities. In children with gingivitis higher detection frequency of some periodontopatic bacteria was detected. Results of the study suggest that some of the investigated gene polymorphisms may play a role in caries development and/or gingivitis.
