Cost analysis of foreskin diseases in children

Abstract:

Tato diplomová práce se věnuje komplexní analýze financování zdravotní péče a nákladového hodnocení vybraných onemocnění předkožky v dětském věku. První část práce poskytuje ucelený přehled modelů financování zdravotní péče, zahrnující systém přímých úhrad, financování z daňových výnosů, veřejné a soukromé zdravotní pojištění, a dále se detailně zaměřuje na různé systémy financování, jako jsou platby za ošetřovací den, metody plateb za výkon a kapitační platby. Zvláštní důraz je kladen na systémy DRG, včetně projektů DRG Restart a IR-DRG. Druhá část práce se zabývá nákladovou analýzou, kde jsou náklady rozčleněny dle druhu, účelu a typu léčby. Zatímco léčba konglutinace probíhá výhradně v ambulantním prostředí a je hodnocena výkonově pomocí bodového systému, na operační řešení fimózy, je pro alokaci nákladů aplikována metoda Activity-Based Costing (ABC), rozšířena o metodu TD - ABC. Výsledky této práce poskytují hlubší pochopení dynamiky nákladů a efektivity různých modelů financování ve zdravotnictví, což může sloužit jako podklad pro zlepšení financování zdravotní péče na oddělení dětské chirurgie.

Abstract:

This thesis is devoted to a comprehensive analysis of health care financing and cost assessment of selected diseases of the foreskin in childhood. The first part of the thesis provides a comprehensive overview of health care financing models, including the direct reimbursement system, tax revenue financing, public and private health insurance, and then looks in detail at different financing systems such as per-care-day payments, pay-for-performance methods, and capitation payments. Special emphasis is placed on DRG systems, including the DRG Restart and IR-DRG projects. The second part of the paper deals with cost analysis, where costs are broken down by type, purpose and type of treatment. While the treatment of conglutination takes place exclusively in the outpatient setting and is evaluated in terms of performance using a point system, for the surgical management of phimosis, the Activity-Based Costing (ABC) method, augmented by the TD - ABC method, is applied for cost allocation. The results of this work provide a deeper understanding of the cost dynamics and efficiency of different healthcare financing models, which can serve as a basis for improving healthcare financing in the Department of Pediatric Surgery.
