Basic principles and utilization of quantitative PCR with respect oncology diagnostics

Abstract:

Polymerasová řetězová reakce (polymerase chain reaction, PCR) je biochemická a molekulárně biologická metoda, která slouží k enzymatické replikaci DNA bez použití živého organismu. Tato metoda se postupem času od svého objevení stala nedílnou součástí laboratoří ať již v oblasti biomedicínské, biologické, ale i archeologické a dalších. Své obliby a používa-nosti dosáhla vzhledem k tomu, že k jejímu provedení teoreticky stačí jediná molekula DNA nebo RNA a není náročná na realizaci. V dnešní době se její použití zaměřuje na detekci zbytkové nemoci (MRD) u onkolo-gických pacientů, u nichž se zjišťuje přítomnost i jen několika rakovinných buněk, které ne-mohou být odhaleny běžnými laboratorními technikami. Testy na MRD mohou zjistit nádor již v jeho počátku a u pacienta, který byl léčen toto zjištění poukazuje na to, že léčba nebyla úspěšná. MRD může tedy odlišit pacienta, který potřebuje intenzivní a eventuálně toxičtější léčbu od toho, který ji nepotřebuje. Obecný předpoklad základu MRD je, že znalost MRD může účinně vést klinickou péči a zvýšit počet vyléčených.

Abstract:

Polymerase chain reaction (PCR) is a method largely applied in biochemistry and molecular biology. Its purpose is enzyme-catalyzed DNA replication without the necessity of presence of any viable organism. This method has since its introduction become a consistent component of laboratories not only in the realm of biochemistry and biology but also archaeology etc. It has gained its popularity and frequency of application due to the fact that only one single molecule of DNA or RNA is required for its completion. It is also notably easy to perform. Today its primary application is aimed at detection of minimal residual disease (MRD) with oncological patients. The proof of the presence of residual cancer cells, that is when only a few cancer cells undetectable by common up-to-date techniques are present, is of highest importance. MRD testing may reveal cancer growth in its early stage and with cancer patients it may suggest the treatment has not been completed. MRD may also predict which patient needs a more aggresive and potentially toxic treatment and which doesn’t. Thus the general point of MRD is to possibly direct the course of treatment and increase the survival rate.
