Nursing process in care of child with Crohn´s disease

Abstract:

Bakalářská práce je zaměřena na ošetřovatelský proces u dítěte s Crohnovou chorobou. Cílem práce je vytvoření podrobného ošetřovatelského plánu péče u vybraného pacienta hospitalizovaného na dětském lůžkovém oddělení po dobu nejméně 3 dnů. Tento plán péče vychází z ošetřovatelských diagnóz NANDA International Taxonomie II a je zaměřen na konkrétní potřeby pacienta. Práce je rozdělena na dvě části, teoretickou a praktickou. Teoretická část je rozdělena na 2 kapitoly. První kapitola popisuje charakteristiku onemocnění, zabývá se klinickým obrazem dítěte a mimostřevními projevy nemoci. Jsou zde popsány příčiny vzniku onemocnění, epidemiologie a vliv nemoci na psychosomatiku dítěte. V neposlední řadě je zde podrobně rozepsaná diagnostika a následná terapie. Druhá kapitola je zaměřena na specifika ošetřovatelské péče a výkony s ní spojené: zavádění nasogastrické sondy, péče o sondu, výživa dítěte a jiné. Praktická část je zaměřena na tvorbu ošetřovatelského plánu péče u 15 letého chlapce, který byl přijat na dětské lůžkové oddělení pro došetření a nastavení terapie. Při tvorbě plánu péče byla využita NANDA International Taxonomie II, odkud byly čerpány veškeré ošetřovatelské diagnózy, na základě kterých byly stanoveny cíle a ošetřovatelské intervence. Po posouzení pacienta bylo stanoveno celkem 9 ošetřovatelských diagnóz, 6 aktuálních a 3 potencionální. Součástí empirické části byl také popis jednotlivých dnů hospitalizace pacienta.

Abstract:

The bachelor thesis focused on the child´s nursing process with Crohn's disease. The aim of the thesis is to create a detailed nursing plan of care for a selected patient hospitalized at the children's inpatient department for at least 3 days. This care plan is based on NANDA International Taxonomy II nursing diagnoses and focuses the patient's specific needs. The thesis is divided into theoretical and practical parts. The theoretical part is divided into 2 chapters. The first chapter describes the characteristics of the disease, deals with the clinical picture of the child and extraintestinal manifestations of the disease. The causes of the disease, epidemiology and the impact of the disease on the child´s psychosomatics are described here. Last but not least, there is a detailed diagnostics and subsequent therapy. The second chapter focuses on the specifics of nursing care and related procedures: nasogastric tube insertion, probe care, child nutrition etc. The practical part focuses on the creation of a nursing care plan for a 15-year-old boy who was admitted to the children's inpatient department for treatment and setting up therapy. NANDA International Taxonomy II was used as a source of all nursing diagnoses in the creation of the care plan. Goals and nursing interventions were determined by this source. After assessing the patient, a total of 9 nursing diagnoses, 6 current and 3 potential were established. A description of individual days of patient´s hospitalization was part of the empirical section as well.
