Subjective evaluation of dyspnea in elderly patients with acute heart failure

Abstract:

Cíl: Diplomová práce se zabývá subjektivním vnímáním dušnosti pacientů hospitalizovaných pro akutní srdeční selhání ve Fakultní nemocnici v Brně na Interní kardiologické klinice na JIP. Cílem práce bylo zmapovat, zda subjektivní hodnocení zdravotního stavu pacientů koreluje s objektivním hodnocením zdravotního stavu lékaři ve skupině pacientů hospitalizovaných pro akutní srdeční selhání. Za pomoci statistických testů zjistit a porovnat rozdíly ve vnímání dušnosti mezi skupinami mladých, starých nebo velmi starých seniorů a zároveň v závislosti na pohlaví. Dále, zda hladina BNP koreluje se subjektivním vnímáním a na závěr zjistit, zda došlo u respondentů ke zlepšení vnímání dušnosti a celkového stavu v poslední den výzkumu. Následně tyto výsledky porovnat s reprezentativními soubory zaměřené na stejné téma v České republice a v zahraničí. Metodika: Naše studie byla zaměřena na pacienty s akutním srdečním selháním. U těchto většinou polymorbidních pacientů jsme sledovali objektivní parametry a především subjektivní vnímání svého stavu. Hodnocení probíhalo po dobu pěti dnů za pomoci dotazníků používaných ve studiích REALAX A TRUE-HF. Testovací soubor tvořilo 20 seniorů. Mezi respondenty bylo celkově 7 žen a 13 mužů. Všichni dotazovaní museli splnit daná kriteria pro zařazení do výzkumu. Výsledky: Z výsledků vyplývá, že subjektivní vnímání seniorů v některých případech koreluje s objektivním hodnocením lékařů. Průměrná hodnota u vnímání dušnosti byla první den 6,71 u všech seniorů a poslední den nastalo zlepšení, kdy byla průměrná hodnota 1,43. Ve všech objektivních parametrech hodnocených lékaři došlo taky ke zlepšení (nejvýraznější byla úprava u ortopnoe ? 1. den byl průměr 3,60 a 5. den byla hodnota 1,45, a pak následovala dušnost, chropy, edém a pulzace). Dále jsme zjistili, že vnímání dušnosti ovlivňuje u respondentů pohlaví a věk. U všech pacientů došlo během pěti dnů ke zlepšení celkového stavu i dušnosti. 1. den uvedlo nejvíce seniorů u dušnosti nejčastěji možnost značně hůře (nejhorší varianta) 55 % a 5. den 77 % seniorů uvedlo nejlepší možnost - značně lépe. Korelace mezi BNP a subjektivním vnímáním dušnosti byla u všech seniorů slabá. Závěr: U pacientů hospitalizovaných pro akutní srdeční selhání po stabilizaci stavu nastalo výrazné zlepšení v oblastech vnímání dušnosti a celkového stavu. U objektivního hodnocení lékaři došlo rovněž ke zlepšení. Dotazníky na hodnocení dušnosti by se měly stát součástí diagnostického postupu u pacientů s akutním srdečním selháním. Dle výsledků dotazníků bychom mohli modifikovat zamýšlený postup léčby s přihlédnutím na subjektivní vnímání hospitalizovaných pacientů.

Abstract:

Aim: This diploma thesis is dealing with a subjective perception of dyspnea in the case of patients hospitalized with acute heart failure which at ICU, Internal Cardiology Department, University Hospital in Brno. The main aim of this work is to verify if there is a correlation between objective assessment of health status by doctors and subjective evaluation of patients hospitalized in the ICU. Differences in the perception of dyspnea between groups of young, old or very old seniors and also in relation to gender are compared by using statistical tests. In the next step a comparison is made, if the level of BNP correlates with subjective perceptions. Finally we are trying to identify if there is an improvement in the perception of dyspnea and overall status in the last day of their hospitalization. Results of this research were compared with similar research in Czech republic and foreign countries. Methodology: This study is aimed at patients with acute heart failure. We have to observed an objective parameter of their health condition and their subjective evaluation that are usually polymorbid. Evaluation was based on questionnaires used in studies REALAX and TRUE-HF for five days. There were 20 seniors (13 men and 7 women) in test group. All respondents having to fulfill the given criteria for inclusion in the research. Results: Results show that the seniors subjective perception correlate in some cases with objective evaluation of doctors. Seniors rated dyspnea by average value 6.71 in the first days of their hospitalization. The average value improved to 1.43 at the end of hospitalization. In all objective parameters evaluated by doctors the improvement was observed. The most significant improvement was observed in the case of othopnea (from 3,6 to 1,45) followed by dyspnoea, rales, edema and pulsations. The perception of breathlessness affects respondents gender and age. Overall condition and dyspnea improved of all patients. The first day 55 % patients assessed their condition as considerably worse and in the fifth 77 % as considerably better. The correlation between BNP and the subjective perception of dyspnea for all seniors was weak. Conclusion: There was observed a significant improvement in the areas of dyspnea and overall state after patients hospitalization with acute heart failure. Doctors have observed significant improvements in their health condition too. Questionnaires for evaluation of dyspnea should become part of the diagnostic process in patients with acute heart failure. Subjective perception of hospitalized patients obtained by a questionnaire should take into account in the case of suitable medical procedure selection.
