Physiotherapy after total knee arthroplasty

Abstract:

Název: Fyzioterapie po TEP kolenního kloubu Cíl: Cílem této bakalářské práce je přispět k řešení problematiky fyzioterapie po totální endoprotéze kolenního kloubu v období hospitalizace a volba optimálních fyzioterapeutických postupů, konceptů a metod u vybraného pacienta. Metodika: Pacienta jsem si vyšetřila den před operací a na základě vstupního vyšetření jsem stanovila krátkodobý fyzioterapeutický plán. Hned po operaci jsem zařadila do cvičební jednotky cévní a dechovou gymnastiku, izometrické cvičení na musculus quadriceps femoris a musculi gluteí. Dalšími fyzioterapeutickými metodami a postupy, které jsem zařadila do rehabilitace, jsou: pasivní a aktivní pohyb, míčková facilitace, techniky měkkých tkání, PNF, Brűgger koncept, Metoda L. Mojžíšové, Spiraldynamic ?, mobilizační techniky, senzomotorická stimulace. V den propuštění pacienta z nemocnice byl proveden výstupní kineziologický rozbor a na jeho základě byl stanoven dlouhodobý fyzioterapeutický plán. Výsledky: Po výstupním fyzioterapeutickém rozboru jsem zjistila zlepšení rozsahů pohybů v kyčelním kloubu do abdukce, flexe, vnitřní a zevní rotace. Nejvíce se zlepšil aktivní pohyb do flexe kolenního kloubu, a to o 10°. Zlepšil se i aktivní pohyb do extenze o 5°. Došlo k nárůstu svalové síly na operované dolní končetině v průměru o 1 stupeň, zmenšení otoku o 5 cm přes stehno 15 cm nad patellou a přes kolenní kloub o 2 cm, zlepšení pohyblivosti inquinální, popliteální a stehenních fasciích, dále došlo k protažení zkrácených svalových skupin. Zlepšili se pohybové stereotypy do extenze a abdukce v kyčelním kloubu. Klíčová slova: Fyzioterapie, kolenní kloub, rehabilitace, totální aloplastika, totální endoprotéza

Abstract:

Title: Physiotherapy after total knee arthroplasty Objective: The objective of this bachelor work is contribute to solving the problems of physiotherapy after total knee arthroplasty in the period of hospitalization and selection the optimal therapeutic methods, concepts and methods in the selected patient. Methods:I examined the patient day before the operation and based on initial examination I set short physiotherapy plan. Immediately after the operation I incorporeted to the exercise unit to do vascular breathing exercises, isometric exercises of musculus quadriceps femoris and musculi glutei. Other methods of physiotherapy procedure which I have included into rehabilitation are: passive and active movement, facilitation with ball, soft tissue techniques, PNF, Brűgger concept, method of L. Mojžíšové, Spiraldynamic ?, mobilization techniques, sensorimotor stimulation. After discharging patient from the hospital was carried final kinesiology analysis and on this was based the long term physiotherapy plan. Results: After the final kinesiology analysis, I found out the improvement of range of motion in the hip joint to abduction, flexion, internal and external rotation. The most improved active movement is the flexion of the knee joint, about 10 °. The active movement to extension of the knee joint was improved too, about 5°. There was an increase around one degree of muscle strenght on operated lower limb and reduction of swelling of 5 cm on the thigh, 15 cm above the patella and around the knee joint 2 cm, then improvement of mobility inguinal, popliteal and femoral fascia, then there was stretch of shortened muscle groups and improvement of the movement stereotypes of extension and the abduction of the hip.
