Physiokinetic therapy protocol for the rehabilitation of a patient with a vertebral burst fracture: clinical case

Abstract:

The spine or vertebral column, is one of the elements of the axile skeleton of the human body together with the bones of the skull and the rib cage. The bones that compose it are called vertebrae which are irregular bones and are twenty-four; to these are added two other bones, the sacrum and the coccyx. The vertebral column is not a rectilinear and rigid structure, but if observed on the lateral plane, the presence of four so-called physiological curves is evident: two lordosis, cervical and lumbar, and two kyphosis, dorsal and sacral. The region of the spine with the highest incidence of vertebral fractures, due to high energy traumatic events, is the dorso-lumbar junction, which represents the passage point from a more rigid region, the dorsal one, which allows flexion-extension movements limited, to a region with greater mobility, the lumbar region. Kyphoplasty is a percutaneous cementoplasty technique that involves the injection of a particular acrylic cement into the vertebral body, trans or extra peduncular, with the use of a balloon that is inserted and inflated inside the vertebral body, via endoscopic, in order to compact the body's bone tissue, creating a cavity where to inject cement, and restoring the height of the vertebra. The clinical case of C., patient undergoing kyphoplasty, following a compression fracture and burst of the vertebral soma of L1, the related rehabilitation protocol and the results achieved will be described.

Abstract:

Il rachide, o colonna vertebrale ? uno degli elementi dello scheletro assile del corpo umano assieme alle ossa del cranio e alla gabbia toracica. Le ossa che la compongono prendono il nome di vertebre che sono delle ossa irregolari e sono ventiquattro; a esse si aggiungono altre due ossa, il sacro e il coccige. La colonna vertebrale non ? una struttura rettilinea e rigida, ma se osservata sul piano laterale, ? evidente la presenza di quattro cosiddette curve fisiologiche: due lordosi, cervicale e lombare, e due cifosi, dorsale e sacrale. La regione del rachide con la maggiore incidenza di fratture vertebrali, dovute a eventi traumatici ad alta energia, ? la giunzione dorso-lombare, che rappresenta il punto di passaggio da una regione pi? rigida, quella dorsale, che consente dei movimenti di flesso-estensione limitati, ad una regione dotata di maggiore mobilit?, quella lombare. La cifoplatica ? una tecnica percutanea di cementoplastica che prevede l'iniezione di un particolare cemento acrilico nel corpo vertebrale, per via trans o extra peduncolare, con l'impiego di un palloncino che viene inserito e gonfiato all'interno del corpo vertebrale, per via endoscopica, al fine di compattare il tessuto osseo del corpo, creando una cavit? dove iniettare il cemento, e ripristinando l'altezza della vertebra. Sar? descritto il caso clinico di C., paziente sottoposto a cifoplastica, in seguito a frattura da compressione e scoppio del soma vertebrale di L1, il relativo protocollo riabilitativo e i risultati raggiunti.
