Amiodarone-induced thyrotoxicosis impact on the patient´s health condition

Abstract:

Cíl: Cílem diplomové práce bylo vypracovat kazuistiku pacienta s onemocněním amiodaronem indukovanou tyreotoxikózou, zejména detailní popsání případu onemocnění, jeho průběhu, komplikací, specifických projevů onemocnění a dopadů nemoci na proměny zdravotního stavu pacienta. Metodika: Praktická část práce je zpracována formou kazuistiky pacienta s výše citovaným onemocněním. Kazuistika detailně popisuje případ onemocnění, jeho průběh, specifické projevy onemocnění i následky léčby amiodaronem, která vyústila v poměrně vzácné komplikace. Výsledky: Amiodaron je antiarytmikum se širokým indikačním spektrem u supraventrikulárních a komorových tachyarytmií, lék má však celou řadu nežádoucích účinků a s ohledem na obsah jodu může významně ovlivňovat funkci štítné žlázy. U pacienta došlo k rozvoji amiodaronem indukované tyreotoxikózy II. typu. Přes veškerou odbornou péči personálu pacient zemřel. Závěr: Amiodaron je třeba indikovat uvážlivě, jelikož jeho vedlejší účinky, zejména u pacientů trpících nějakou ze srdečních chorob, mohou mít fatální následky, které se u pacienta objevily. Veškerá péče nejen ošetřujícího personálu, ale i péče mezioborová, byla na velmi vysoké úrovni. I přesto, že péče byla prováděna každý den se stejnou intenzitou a důležitostí, nepodařilo se pacienta zachránit.

Abstract:

Objective: The aim of the thesis was to elaborate a case study of a patient with the amiodarone-induced thyrotoxicosis disease, in particular, a detailed description of the case, course of the disease, complications, specific manifestations of the disease and its on the patient's conditions. Methodology: The practical part is made as a study of a patient with the above cited diseases. The case study describes in detail the disease, its progression, disease specific symptoms and effects of amiodarone treatment, which resulted in a relatively rare complication. Results: Amiodarone is an antiarrhythmic agent with a broad spectrum indicator of supraventricular and ventricular tachyarrhythmia, but the drug has a wide range of side effects as well and with respect to iodine content may significantly affect thyroid function. The patient had developed amiodarone-induced thyrotoxicosis of type 2. Despite all care the patient died. Conclusion: It is necessary to indicate amiodarone prudently, because its side effects, particularly in patients suffering from a heart disease, can be fatal, as were in this case study. All provided care, not only of nursing staff but also interdisciplinary care, was at a very high level. Even though the treatment was carried out every day with the same intensity and importance, could not save the patient.
