Is algorithm of the examination of predictive factors affecting healing of the lover limbs defect at the present level sufficient?

Abstract:

Úvod: Nehojící se ulcerace nohy můžou být velmi různorodé etiologie. Přesná identifikace příčiny chronické rány je důležitá pro plánovaní další terapie, aby byly minimalizovány komplikace vznikající v důsledku perzistujícího defektu. Cíl práce: Analyzovat dostupné doporučené postupy a algoritmy v diagnostice a léčbě nehojících se ran nohy. V rámci prospektivní studie následně ověřit, jak se liší proces hojení u pacientů s rozdílným typem rány na noze, a zda existuje souvislost mezi diagnostickými metodami a procesem hojení. Praktickým výstupem práce je klinicky využitelný algoritmus vyšetření pacienta s nehojící se ranou nohy. Metodika: Do klinické prospektivní studie (leden 2011 až prosinec 2013) bylo zařazeno 72 pacientů s nehojící se ranou na noze různé etiologie, kteří podstoupili sérii běžných vyšetření v souvislosti se základní identifikací příčiny defektu a plánováním terapeutického postupu. Statistická analýza dat byla provedena pomocí statistického programu SPSS verze 18 (Kruskal Wallis test, Mann Whitney test a Pearsonova Chí-kvadrátu) na hladině významnosti α 0.05. Výsledky: Z analýzy odborných zdrojů vyplývá, že v ČR je oblast doporučených postupů nedostatečně zpracována. Dle výsledku provedené studie bylo zjištěno, že existuje statisticky významný vztah mezi lokalizací rány a komorbiditami (p 0,000), typem rány a hodnotou ankle brachial indexu (p 0,005), hodnotou ankle brachial indexu a průchodností cév dolní končetiny dle vyšetření CT angiografie (p 0,014 – 0,004). Významná statistická závislost byla také prokázaná mezi délkou hojení a hlavní komorbiditou (p 0,016) a mezi délkou hojení a provedení cévní intervence (p 0,044). Histologicky potvrzena mikroangiopatie naopak nebyla ověřena jako faktor statisticky významně predikující délku hojení rány. Závěr: Proces léčby nehojících se ulcerací nohy není v ČR jednoznačně standardizován. Na základě provedené studie lze říci, že v diagnostickém procesu je důležitá lokalizace defektu, která souvisí s hlavní komorbiditou a tak je možné stanovení typu ulcerace. Hlavní komorbidita se také ukázala jako významný prediktivní faktor v délce hojení chronických ulcerací. Ankle brachial index je možno použít jako základní vyšetřovací metodu v diagnostice ischemické choroby dolních končetin, která koreluje s průchodností tepen dle CT angiografie. V případě provedené cévní intervence – perkutánní angioplastiky je délka hojení rány delší, což souvisí s horším stavem končetiny v důsledku ischemie. Na základě ověřených dat z provedené studie a analýzy odborných zdrojů byl vytvořen jednoduchý klinický algoritmus pro diagnostiku a terapii nehojících se nohy.

Abstract:

Introduction: Non-healing foot ulcers can be of very diverse etiology. Precise identification of the cause of the chronic wound is important for further therapy planning in order to minimize complications resulting from persisting ulcer. Objective: To analyse the available recommended treatment guidelines and algorithms in the diagnostics and treatment of non-healing foot wounds. In the prospective study, to determine the differences among the healing processes of patients with a different type of foot wound, and to determine whether there is a correlation between the diagnostic methods and the healing process. The practical outcome of this work is a clinically applicable algorithm of examination of patients with non-healing foot wound. Methodology: A clinical prospective study (January 2011 to December 2013) included 72 patients with a non-healing foot wound of different etiology. The patients underwent a series of common examinations in connection with the primary identification of the ulcer cause and therapeutic procedure planning. Statistical data analysis was performed using the statistical program SPSS version 18 (Kruskal Wallis test, Mann Whitney test and Pearson's Chi-square test) at significance level α 0.05. Results: The analysis of expert sources shows that in the Czech Republic the recommended treatment guidelines are inadequately processed. According to the result of the conducted study it was found out that there is a statistically significant relation between the wound localization and comorbidities (p 0.000), the type of the wound and the value of ankle brachial index (p 0.005), the value of ankle brachial index and the blood vessel patency of the lower limb by CT angiography (p 0.014 – 0.004). A significant statistical relation was also proved between the duration of healing and major comorbidity (p 0.016), and the duration of healing and performed vascular intervention (p 0.044). However, histologically confirmed microangiopathy was not validated as a statistically significant factor predicting the duration of wound healing. Conclusion: The treatment process of non-healing foot ulcers is not clearly standardized in the Czech Republic. The performed study shows the importance of the ulcer localization in the diagnostic process, related mainly to the comorbidity and enabling to determine the type of ulceration. The main comorbidity also emerged as an important predictive factor in the duration of healing of chronic ulcers. Ankle brachial index can be used as a basic examination method in the diagnostics of peripheral vascular disease of lower limbs, which correlates with arterial patency by CT angiography. If performed vascular intervention – percutaneous angioplasty, the duration of wound healing is longer, which is related to a worse condition of the limb due to ischemia. Based on the verified data of the conducted study and expert sources analysis, a simple clinical algorithm was created for the diagnostics and treatment of non-healing foot.
