Rehabilitation with the syndrome of painful shoulder

Abstract:

ABSTRAKT Název: Fyzioterapie u syndromu bolestivého ramene. Cíl: Cílem bakalářské práce je přispět k řešení problematiky syndromu bolestivého ramene volbou optimálních fyzioterapeutických postupů, metod a konceptů. Metodika: V této práci je zpracována kazuistika pacientky rok po zlomenině chirurgického krčku pažní kosti s abrupcí velkého hrbolu. Součástí je vstupní i výstupní kineziologický rozbor a speciální testy ramenního kloubu. Na základě vyšetření byl sestaven krátkodobý fyzioterapeutický plán. Fyzioterapeutická intervence trvala 2 měsíce. Během 15 návštěv byly využity různé koncepty a metody: mobilizace, TMT, ruční trakce, PNF, cvičení v otevřených i uzavřených řetězcích, cvičení dle R.Brunkow, ACT, dechová cvičení a cvičení s pomůckami (tyč, overball, gymball, Propriomed, balanční plochy). Cvičení bylo doplněno fyzikální terapií. Výsledky: Z výstupního rozboru je patrné výrazné zlepšení funkce celého ramenního pletence, mimo jiné zvětšení rozsahu pohybu i svalové síly. Došlo ke zklidnění klidových bolestí ramenního kloubu i bolestí hrudní páteře. Závěr: Zlepšení celkového stavu se stalo pro pacientku motivací pro dlouhodobější autoterapii a změnu životního stylu. Pro mě se tato práce stala přínosem pro další praxi a obohatila mě o nové poznatky.

SUMMARY: Name: Physiotherapy of pain arm syndrome Aim: The aim of this bachelor thesis is to support how to solve problems connected to the syndrome of painful shoulder by choosing optimal physiotherapeutic procedures, methods and variety of concepts. Methodology: This thesis is based on the patient's case study, which was processed one year after the fracture of the surgical neck of the femur with the abdomen of the big bump. It includes input and output kinesiological analysis and special shoulder joint tests. Based on the physical examination, a shortterm rehabilitation plan was drawn up. The physiotherapeutic intervention lasted for 2 months. During 15 meetings various concepts and methods have been used: mobilization, TMT, hand traction, PNF, open and closed chain exercises, exercises by R. Brunkow, ACT, breathing exercises and exercises with aids (rod, overball, gymball, propriomed, flat). Exercise has been supported by physical therapy. Results: The output analysis shows a significant improvement in the function of the whole arm shoulder, furthermore increase of the range of motion and muscle strength. Restless pains of the shoulder joint and the chest spine have been calmed down. Conclusion: The improvement of the patient's overall condition has motivated her lifestyle changes and ongoing self-treatment. For me, working on this thesis has provided new knowledge and will be an asset for further practice.
