Immunohistochemical determination of L1CAM and p53 in endometrial carcinoma

Abstract:

Zhoubné nádory těla děložního jsou nejčastější gynekologickou malignitou v ČR. Karcinomy endometria rozlišujeme na dva základní typy – typ I, hormonálně dependentní a typ II, na hormonech nezávislé. V nádorové diagnostice se hojně uplatňují IHC metody, které nám umožňují lokalizovat určité antigeny a zjistit jejich expresi. V posledních letech se začalo využívat stanovení L1CAM a p53 jako prediktivních markerů. Mutace p53 a overexprese L1CAM jsou pro karcinom endometria charakteristické a jsou spojovány s horší prognózou. Na začátku teoretické části je ve zkratce popsána anatomická a histologická stavba dělohy. Následuje charakteristika samotného nádorového onemocnění se zaměřením na karcinom endometria. Další část je věnována imunohistochemii, je zde objasněn princip IHC reakcí a základních metod. Ke konci jsou popsány kontroly vzorků, kontrola kvality a několik nejčastějších chyb celého procesu laboratorního vyšetření. Praktická část obsahuje detailní popis celého procesu zpracování vzorku a použité IHC metody, pracovní protokol ručního i automatického stanovení a vyhodnocení výsledků.

Abstract:

Malignant tumors of the uterine body are the most common gynecological malignancy in the Czech Republic. Endometrial carcinomas are classified as type I, hormone-dependent and type II, hormone-independent. In tumor diagnostics, IHC methods are abundantly used to locate certain antigens and detect their expression. In recent years, L1CAM and p53 have been used as predictive markers. p53 mutations and L1CAM overexpression are characteristic of endometrial cancer and are associated with a worse prognosis. At the beginning of the theoretical part, the anatomical and histological structure of the uterus is briefly described. The following is a characteristic of the cancer itself with a focus on endometrial cancer. The next part is devoted to immunohistochemistry, the principle of IHC reactions and basic methods is clarified. At the end, controls, quality control, and some of the most common errors in the entire laboratory process are described. The practical part contains a detailed description of the whole sample processing and used IHC method, manual and automatic protocol and evaluation of the results.
