Determination of presepsin in neonates and its difficulties

Abstract:

Úvod: Tato bakalářská práce je zaměřena na stanovení hladin presepsinu v plazmě u novorozenců. Zkoumá presepsin jako nadějný marker sepse a porovnává jej s ostatními markery, konkrétně C-reaktivním proteinem (CRP) a interleukinem-6 (IL 6). Také se detailněji zaměřuje na analyzátor PATHFAST. Metodika: Bylo vyšetřeno 132 vzorků plazmy novorozenců zaslaných na Oddělení klinické biochemie FNO s požadavkem na stanovení koncentrace presepsinu, CRP a IL-6. Hladina CRP byla měřena na biochemickém analyzátoru AU 5800 a hladina IL-6 na imunochemickém analyzátoru ADVIA Centraur XPT. Měření presepsinu probíhalo na analyzátoru PATHFAST, který k analýze využívá principu enzymatické imunoanalýzy. Výsledné hodnoty presepsinu u novorozenců byly porovnány s koncentracemi zbylých markerů. U 13 pacientských vzorků plazmy z laboratoře ve Frýdku-Místku proběhlo mezilaboratorní porovnání presepsinu s naší laboratoří. Statistické zpracování dat bylo provedeno v softwaru MedCalc a MS Excel. Výsledky: Studovaný soubor tvořilo 24 novorozenců bez infekce, 39 novorozenců s adnátní infekcí, 34 nezralých novorozenců a 35 novorozenců s jinou diagnózou. Závislost jednotlivých zánětlivých markerů (CRP, IL-6 a presepsinu) na typu diagnózy byla hodnocena pomocí analýzy rozptylu (Kruskal-Wallisův test), statistická významnost byla zvolena na hladině ?=0,05; pomocí krabicových grafů a Post-hoc analýzy Dunnovou metodou. Protože data nevykazovala normalitu pro vyhodnocení vzájemné korelace, byl použit neparametrický Spearmanův korelační koeficient a byla prokázána statisticky významná korelace mezi gestačním týdnem a porodní hmotností (rs=0,881; P<0,0001), mezi CRP a porodní hmotností (rs=0,474; P<0,0001), mezi CRP a gestačním týdnem (rs=0,485; P<0,0001) a mezi IL-6 a presepsinem (rs=0,193; P<0,05). Pro porovnání počtu pozitivních a negativních hodnot presepsinu (cut-off 643 ng/L) a IL-6 (cut-off 4,4 ng/L, 20 ng/L, 30 ng/L, 40 ng/L a 50 ng/L) na typu diagnózy byl použit chí-kvadrát test. Pomocí Kappa statistiky bylo prokázáno, že korelace a shoda presepsinu a IL-6 roste s jeho zvyšujícím se cut-off, korelace byla významná až u cut-off IL-6 50 ng/L (?=0,194; 95% CI 0,024-0,364). Mezilaboratorní porovnání vzorků bylo vyhodnoceno pomocí neparametrické regrese dle Passing-Babloka a Bland-Altmanova diferenčního grafu. Výsledky byly statisticky shodné. Závěr: Presepsin je vhodným markerem k diagnostice časné novorozenecké sepse. Jako jediný z porovnávaných markerů dokázal nejlépe rozlišit novorozence s infekcí od novorozenců bez infekce, s nezralostí a s jinou diagnózou na rozdíl od IL-6 a CRP. V budoucnu je potřeba se zaměřit na novorozence s pozdní novorozeneckou sepsí, ověřit referenční rozmezí pro novorozence u presepsinu a stanovit vhodnou cut-off hodnotu IL-6 pro novorozence.

Abstract:

Introduction: This bachelor thesis is focused on the determination of plasma levels of presepsin in neonates. It examines presepsin as a promising marker of sepsis and compares it with other markers, specifically C-reactive protein (CRP) and interleukin 6 (IL-6). The thesis also focuses on the PATHFAST analyzer in more detail. Methods: 132 neonatal plasma samples, sent to the Department of Clinical Biochemistry of the FNO with the request to determine concentration of presepsin, CRP and IL-6, were examined. The CRP level was measured using AU 5800 biochemical analyzer and the IL-6 level using ADVIA Centraur XPT immunochemical analyzer. Measurement of presepsin level was carried out on PATHFAST analyzer, which measuring principle is based on enzymatic immunoassay. The measured presepsin levels in neonates were compared with concentrations of the remaining markers. An interlaboratory comparison of presepsin levels was performed on 13 patient plasma samples from the laboratory Frýdek-Místek with our laboratory. Statistical data processing was carried out using MedCalc and MS Excel software. Results: The studied group consisted of 24 newborns without infection, 39 newborns with a congenital infection, 34 premature newborns and 35 newborns with other diagnosis. The dependence of individual inflammatory markers (CRP, IL-6 and presepsin) on the type of diagnosis was assessed by analysis of variance (Kruskal Wallis test) at a significance level of ?=0.05; using box plots and a post-hoc analysis (Dunn's method). Considering that data did not show normality for correlation analysis, the nonparametric Spearman correlation coefficient was used and statistically significant correlation was demonstrated between gestational week and birth weight (rs=0.881; P<0.0001), between CRP and birth weight (rs=0.474; P<0.0001). , between CRP and gestational week (rs=485; P<0.0001) and between IL-6 and presepsin levels (rs=0.193; P<0.05). A chi-square test was used to compare the number of positive and negative levels of presepsin (cut-off 643 ng/L) and IL-6 (cut off 4.4 ng/L, 20 ng/L, 30 ng/L, 40 ng/L and 50 ng/L) per type of diagnosis. Using a Kappa statistic, the correlation and concordance between presepsin and IL-6 increase with its increasing cut-off value, the correlation was significant only for the IL-6 cut-off 50 ng/L (?=0.194; 95% CI 0,024-0,364). Interlaboratory comparison of samples was evaluated using Passing-Bablok nonparametric regression and Bland-Altman difference plot. The results were statistically equivalent. Conclusion: Presepsin is a suitable marker for the diagnosis of early neonatal sepsis. In contrast to IL-6 and CRP, presepsin was the only one of the compared markers able to best differentiate newborns with infection from newborns without infection, with prematurity and other diagnoses. In the future it may be neccessary to focus on newborns with late neonatal sepsis, to verify a neonatal reference range for presepsin, and to determine an appropriate cut-off value of IL-6 for neonates.
