Nursing Process in Care of a Patient with a Stoma

Abstract:

Bakalářská práce je zaměřená na ošetřovatelský proces v péči o pacienta se stomií. Tato práce se týká pacientky, které byla nově zavedená trvalá jednohlavňová sigmoideostomie. Důvodem k její založení byl diagnostikovaný adenokarcinom rekta. Cílem bakalářské práce je vypracovat ošetřovatelský plán vycházející z ošetřovatelských diagnóz NANDA-I Taxonomie II, které se vztahují ke konkrétním potřebám pacienta se stomií (včetně určujících znaků, souvisejících faktorů, rizikových faktorů, cíle, plánu, konkrétních intervencí, realizace ošetřovatelské péče a hodnocení). Bakalářská práce je rozdělená na teoretickou a praktickou část. Teoretická část je rozdělená na dvě hlavní kapitoly. V první kapitole jsou popsány jednotlivé druhy stomií, indikace k založení stomie, jednotlivé komplikace stomií, stomické pomůcky, postup jejich výměny, psychická podpora a život se stomií. V druhé kapitole je popsána ošetřovatelská péče o pacienta se stomií. Je zde popsána příprava pacienta k operaci, intraoperační péče a pooperační péče. V praktické části bakalářské práce se nachází vypracovaný ošetřovatelský plán péče, který je zaměřený na konkrétní potřeby pacienta se stomií. Na základě určujících znaků, souvisejících faktorů a rizikových faktorů byly určeny ošetřovatelské diagnózy, které jsou zpracovány podle klasifikačního systému NANDA-I Taxonomie II. K jednotlivým ošetřovatelským diagnózám byl stanovený cíl péče. Rovněž byly navrženy konkrétní intervence, které vedou ke zdárnému splnění stanoveného cíle. Také byla popsána realizace ošetřovatelské péče a její hodnocení.

Abstract:

The Bachelor's thesis focuses on the nursing process involved in caring for a patient with a stoma. This thesis refers to a patient who has been recently subjected to a permanent single-headed sigmoidectomy. It was instituted due to a diagnosis of rectal adenocarcinoma. The presented Bachelor's thesis aims at determining a nursing plan based on the NANDA-I Taxonomy II nursing diagnoses pertaining to the specific needs of the ostomy patient (including determinants, associated factors, risk factors, goals, plan, particular interventions, implementation of nursing care and evaluation). The Bachelor's thesis is divided into theoretical and practical parts. The theoretical part consists of two main chapters. The first chapter discusses the different types of stomas, indications for instituting a stoma, individual complications of stomas, ostomy instruments, the procedure for their replacement, psychological support, and life with a stoma. The second chapter covers the topic of nursing care for a patient with a stoma. It describes the preparation procedure for surgery, intraoperative and postoperative care. The practical part of the Bachelor's thesis contains the developed nursing care plan, directed to the specific needs of the stoma patient. Nursing diagnoses were determined and processed according to the NANDA-I Taxonomy II classification system based on determinants, associated and risk factors. A care goal was set for each nursing diagnosis. Specific interventions were also suggested that would result in the successful achievement of the stated goal. The implementation of nursing care and its evaluation were also described.
