Sexual dysfunction in women with epilepsy

Abstract:

Zatímco v minulých desetiletích byla na poli výzkumu příčin sexuálních poruch u epilepsie věnována pozornost zejména jejich souvislosti s častěji pozorovanými hormonálními poruchami a chronickou antiepileptickou medikací, s rostoucím stupněm poznání integrační podstaty procesů cerebrálních sítí a jejich patofyziologie přibývá nepřímých i přímých důkazů o dominantním významu intaktní struktury a funkce rozsáhlých mozkových oblastí. Teprve nečetné recentní práce včetně naší zjišťují souvislost sexuálních poruch u epilepsie s mírou deprese bez ohledu na hladiny hormonů nebo vlastní epileptické záchvaty. Přestože je deprese velmi častou komorbiditou epilepsie a přestože výsledky některých studií naznačují dokonce významnější negativní dopad deprese na kvalitu života pacientů a pacientek s epilepsií, než je dopad vlastních epileptických záchvatů, zůstává deprese v běžné praxi často nediagnostikována a neléčena.. Důvodem je dosud velmi malá obecná zkušenost s diagnostikou a léčbou deprese u pacientů s epilepsií a nedostatek evidence-based poznatků ohledně účinnosti a bezpečnosti antidepresiv u pacientů a pacientek s epilepsií. Vzhledem k referovanému negativnímu či pozitivnímu působení některých antidepresiv na sexuální funkce zůstává tedy otázka možnosti léčby sexuálních poruch antidepresivy výzvou pro budoucí klinický výzkum.

Abstract:

In past decades, attention was directed mainly to the association between sexual dysfunction in epilepsy patients and hormonal abnormalities or chronic antiepileptic medication use. With the increasing knowledge of the integrational principle of cerebral network processes and their pathophysiology, there is growing evidence of the dominant role of the intact cerebral structure and its function in human sexuality. Only occasional recent studies, including our own work, have started to direct attention to the correlation between sexual dysfunction in epilepsy and comorbid depression, with almost no concurrent association found between sexual functions and hormones or seizure frequency. Despite the high prevalence of depression in epilepsy, and despite the fact that according to some studies depression has an even greater impact on the epileptic patient's quality of life than the seizures themselves, depression is still under-recognized and undertreated in common practice. The reason is that there is little experience with diagnosing and treating depression in epilepsy patients, and a lack of controlled studies proving both the efficacy and safety of antidepressants in the epilepsy population. Moreover, the known side effects of some antidepressants on sexual function have to be considered. The potential of treating sexual dysfunction with antidepressants presents a challenge for future clinical research.
