Degenerative Cervical Spinal Cord Compression: Diagnostics, Pathophysiology, Clinical Manifestation, and Practical Management

Abstract:

Degenerativní cervikální myelopatie (DCM), je nejzávažnějším důsledkem spinální cervikální stenózy a je nejčastější příčinou postižení krční míchy a paraparézy dolních končetin u jedinců starších 55 let. Klíčovým mechanismem rozvoje DCM je mechanická komprese krční míchy, kdy řadou patofyziologických procesů dochází až ke klinickým projevům myelopatie. Mícha je však poměrně značně rezistentní k mechanické kompresi. Řada jedinců tak může být dlouhou dobu kompletně asymptomatických nebo mají pouze lokalizované segmentové bolesti či projevy krčních radikulopatie, avšak bez jasných myelopatických klinických projevů, tzv. nemyelopatická degenerativní cervikální míšní komprese (NMDCC). Současná prevalence cervikální stenózy a NMDCC není známa a v literatuře se často liší. Diagnostika DCM je založena na klinických známkách DCM a nálezu komprese na MRI. Klinické známky jsou zejména: bolesti/ztuhlost krční páteře, neobratné ruce, slabost, parestezie horních končetin, zhoršení chůze a Lhermitheův příznak. Autonomní symptomy zahrnují močovou urgenci a inkontinenci. Z radiologických metod se nejčastěji používá nativní rentgen, MRI, event. CT vyš. MRI je nejpřínosnější pro diagnostiku DCM, protože nejlépe zobrazí konturu míchy a míšních kořenů. Jakákoliv deformace míchy oproti normálu nebo cirkulární zúžení (např. i mediálním výhřezem disku) by mělo být považováno jako možný zdroj neurologické dysfunkce. V poslední době se stále více prosazují nové MRI techniky jako difuzní MRI a magnetická spektroskopie, kterou budou pravděpodobně hrát kritickou roli v rozhodování indikace k operačnímu řešení. Současná doporučení pro management pacientů s DCM stanovují nová doporučení. U pacientů se střední a těžkou DCM je doporučena chirurgická intervence. Neexistuje však jasné „evidence based“ doporučení k léčbě pacientů s mírnou DCM a NMDCCC. Ty záleží na identifikaci specifických klinických, radiologických či elektrofyziologických markerů, které vedou k progresi do symptomatické DCM. V současné době již byla řada těchto prediktorů identifikována.

Abstract:

Degenerative cervical myelopathy (DCM) is the most serious consequence of cervical spinal canal stenosis and represents the most frequent cause of cervical spinal cord (CSC) injury and lower paraparesis in individuals older than 55 years. Mechanical degenerative cervical spinal cord compression (DCSCC) is a key mechanism for the development of DCM, which along with a complex of pathophysiological mechanisms leads to a variety of myelopathy symptoms. The spinal cord, however, is quite resistant to the mechanical compression that may cause DCM. Subjects with cervical cord compression may thus remain completely asymptomatic or develop axial pain or signs of root compression, but without clear signs of symptomatic myelopathy - “non-myelopathic degenerative cervical cord compression” (NMDCCC). The current prevalence of cervical stenosis (CS) and NMDCCC is not known and data in the literature differ widely. Diagnosis of DCM is based on clinical findings of cervical myelopathy and magnetic resonance imaging (MRI) signs of DCSCC. The initial identification of DCM is based heavily on the presence of symptoms: neck pain or stiffness, numb and/or clumsy hands, weakness, bilateral arm paraesthesia, gait impairment, and/or Lhermitte’s sign. Autonomic symptoms include increased urinary urgency and frequency and urinary incontinence, which are typically not the first symptom noted by the individual but present in conjunction with other symptoms. Imaging modalities for cervical spondylosis aim to assist the clinician in differentiating discogenic neck pain, radiculopathy, and myelopathy. Radiological assessment helps to localise the site and level of the disease for preoperative planning when surgical intervention is required. The current modalities in common use are plain film roentgenology, magnetic resonance imaging, and computed tomography. MRI is the most useful imaging investigation for DCM diagnosis, because it clearly shows the outline of the spinal cord and nerve roots in relation to surrounding cerebrospinal fluid (CSF). Any deformation of the spinal cord from its normal shape, such as flattening, indentation, torsion, or circumferential narrowing due to adjacent tissues (a bulging disc) should be considered a type of compression that may or may not cause neurological dysfunction. Recently, new advanced MRI techniques allow the quantifying of microstructural changes and may offer diagnostic measures and predictors of long-term outcomes. Diffusion MRI (dMRI) and proton single voxel magnetic resonance spectroscopy (MRS) can elucidate details of the microstructural and neurochemical organization of the CSC. A desirable diagnostic tool will provide high sensitivity for the detection of early CSC changes and will overcome the diagnostic uncertainties of standard MRI techniques. This will have critical consequences for surgical decision-making in early or mild cases of DCM, to respond to continuous debate regarding the benefits vs. risks of surgical intervention. Despite suggested relevance in degenerative CSC alteration, application of advanced MRI techniques in the CSC is extremely challenging. Contemporary recommendations for management of mild, moderate, or severe DCM is provided by new guidelines. In patients with moderate and severe DCM surgical intervention is strongly recommended. However, in patients with mild DCM and NMDCCC, there is no clear, evidence-based agreement on the optimum management and treatment algorithm. It depends upon the development or identification of sensitive and specific clinical, radiological, and/or electrophysiological markers that could reliably predict progression to symptomatic DCM. Currently, a lot of predictors of such involvement have been identified and this has led some surgeons to recommend decompression surgery in these high-risk patients. However, further studies are required to refine our understanding of the frequency, timing, and predictors of myelopathy development in NMDCCC patients
