Women´s perspectives on the quality of perinatal care

Abstract:

Cíl: Cílem práce bylo zmapovat spokojenost žen s perinatální péči poskytovanou v porodnických zařízeních v Ostravě. Metodika: V období od května do srpna roku 2014 bylo prováděno výzkumné šetření pomocí dotazníku ve třech porodnických zařízeních v Ostravě. Sběr dat probíhal na oddělení šestinedělí. Výsledný soubor respondentek byl tvořen 413 ženami, z toho bylo 199 prvorodiček a 214 vícerodiček. Tyto ženy v porodnickém zařízení porodily a následně byly hospitalizovány na oddělení šestinedělí minimálně po dobu tří dnů. Výsledky: Na porodním sále byla zaznamenána nejvyšší spokojenost v dimenzi fyzické pohodlí a služby a nejnižší v dimenzi kontrola rodičky a její podíl na rozhodování. Na oddělení šestinedělí byla nejlépe hodnocena dimenze kontrola ženy a její podíl na rozhodování a nejhůře dimenze nadřazenost a nedostatek zájmu. Položka s nejvyšším hodnocením se týkala čistoty prostor na porodnickém oddělení a položka s nejnižším hodnocením se vztahovala ke spokojenosti se stravou. S perinatální péči byly signifikantně spokojenější ženy rodící v nefakultních nemocnicích, ženy rodící vaginální cestou, vícerodičky a ženy hospitalizovány na nadstandardních pokojích. Závěr: V perinatální péči je důležitý individuální a respektující přístup zdravotníků, podávání kvalitních informací a posílení postavení rodiček v systému porodnické péče.

Abstract:

Objective: The goal of the thesis was to explore the satisfaction of women with the perinatal care that is provided by the maternal hospitals in Ostrava. Methodology: There was conducted the survey by using the questionnaire in 3 maternal hospitals in Ostrava from May till August 2014. Data gathering was performed at the confinement ward. Final set of the respondents was created by 413 women, whereas 199 out of them were primiparas and other 214 were multigravidas. Those women had given birth in the maternal hospital and were hospitalized at the confinement ward for at least 3 days. Findings: There was the highest level of satisfaction registered in the area of physical comfort and service in the delivery room. The lowest satisfaction was detected in the area of supervision of the woman in childbed and her share in decision making process. The dimension of woman?s surveillance and their share by making decisions was assessed as best at confinement ward. Aspect of superiority and lack of interest was ranked as the worst one. Item with the highest evaluation was related to cleanliness of the premises at maternal hospital and the attribute with the lowest evaluation referred to satisfaction with the food. The women giving birth in non-teaching hospital, giving birth in vaginal way, multigravidas and women hospitalized at above standard rooms were significantly more satisfied with the perinatal care. Conclusion: The individual and respecting approach of the medical staff, providing of relevant and good- quality information and empowerment of the women?s position in the system of obstetric care is crucial in the perinatal care.
