The nursing process of woman with premature labor

Abstract:

Bakalářská práce se zabývá problematikou předčasného porodu, což je v současné době velmi aktuální téma, zejména díky narůstající tendenci těchto porodů, ale také vzhledem k faktu, že se významně podílí na novorozenecké morbiditě a mortalitě. Rovněž je v těchto případech ohroženo zdraví a psychický stav matky. Bakalářská práce je proto zaměřena na ošetřovatelský proces u ženy s předčasným porodem, přičemž cílem práce je vytvoření ošetřovatelského procesu, který by mohl být užitečný přímo v praxi pro porodní asistentky. V teoretické části práce je popsána problematika předčasného porodu, jeho příčiny, komplikace a léčba. Dále je věnována pozornost řešení ukončení těhotenství - kde a jak jsou předčasné porody vedeny. V neposlední řadě je řešen psychologický aspekt předčasného porodu. V praktické části práce je popsán případ konkrétní ženy, která předčasně porodila ve 34. týdnu těhotenství. Žena byla během těhotenství sledována pro inkompetentní hrdlo děložní a následně hospitalizována z důvodu předčasného odtoku plodové vody na gynekologicko-porodnickém oddělení Fakultní nemocnice Ostrava. Z informací od zdravotnického personálu, ze zdravotnické dokumentace, ale taktéž za pomoci nestandardizovaného rozhovoru přímo s ženou byl následně sestaven ošetřovatelský proces, který se skládá ze zhodnocení ošetřovatelské anamnézy v jednotlivých NANDA doménách, ze kterých bylo následně vytvořeno 7 aktuálních a 5 potencionálních ošetřovatelských diagnóz. Ošetřovatelské diagnózy jsou vytvořeny pomocí klasifikace NANDA I, taxonomie II.

Abstract:

This bachelor thesis deals with problems of a premature birth, which is very actual topic in the present time. Primarily thanks to the growing tendency of these births, but also in view of the fact, that it significantly takes part in a new-born morbidity and mortality. In these cases isalso threatened the health and psychic condition of the mother. The bachelor thesis is therefore aimed on a nursing process by a woman with the premature birth, whereas the aim of this thesis is to create a nursing process, which could be useful directly in the practise for midwife. The theoretical part of the thesis is aimed on the issue of the premature birth, its cases, complications and treatment. Then the attention is dedicated to resolution of ending pregnancy - where and how are premature births led. Last but not least the psychic aspect of the premature birth is solved in the thesis. The practical part of this thesis is aimed on a concrete woman, which had the premature birth in the thirty - fourth week of the pregnancy. The woman was monitored during the pregnancy for incompetent uterine throat and then hospitalized due premature drainage of amniotic fluid at the gynaecological and obstetric department of the Faculty Hospital Ostrava. Thanks to the information from the medical personnel, medical documentation, but also with help of non-standardized conversation directly with the woman, was after that compiled the nursing process. The nursing process, which consist of evaluating the nursing anamnesis in NANDA domains, from which was the created seven actual and five potential nursing diagnoses. The nursing diagnoses are created with the help of classification NANDA I, taxonomy II.
