The History of Trauma and Dissociative Experience in Girls

Abstract:

Dizertační práce je souborem deseti studií zabývajících se traumatickou zkušeností a disociativním prožíváním dívek. Autorčiny více než dvacetileté odborné zkušenosti v psychiatrické a psychoterapeutické praxi se promítly i do jednotlivých článků, v nichž se prolínají klinická a výzkumná témata. Dizertace obsahuje přehledové články, případové studie, kvalitativní analýzy textu, i kvantitativní výzkum vztahu mezi sexuálním traumatem, disociací a sebevražednými tendencemi. K nejvýznamnějším výsledkům patří zjištění týkající se dalekosáhlého dopadu prožitého traumatu v raném dětství na rozvoj pozdější psychopatologie jak v dětství, tak v dospělosti, včetně jeho vlivu na somatické zdraví v celoživotní perspektivě. Rané trauma, zvláště sexuální zneužívání dívek, má přímou souvislost s rozvojem disociativního prožívání. Zjistili jsme, že takto traumatizované adolescentní dívky mají tendenci se fyzicky poškozovat, jsou náchylné k suicidálním ideacím i sebevražedným pokusům. Výsledky mediační analýzy naznačují, že dívky, které mají zkušenost se sexuálním zneužíváním, vykazují suicidální tendence především tehdy, když se u nich vyskytne též disociativní symptomatologie. Po identifikaci příznaků disociativní poruchy by měla být zahájena intenzivní psychoterapeutická péče, neboť je zde vysoké riziko výskytu sebedestruktivních či sebevražedných tendencí. Psychoterapie traumatizovaných jedinců by se měla zaměřit na zmírnění disociativního prožívání. Ukazuje se, že mezi účinné specializované postupy patří externalizace obtíží s využitím expresivních, imaginativních a metaforických vyjádření, které umožňují kontejnovat nesnesitelné, nepřestavitelné, někdy i nevyslovitelné zkušenosti spojené s traumatem. Otevírá se tak prostor pro jejich přeměnu a úzdravný proces traumatizovaného jedince. Celková informovanost o problematice disociativních poruch se u odborníků zabývajících se duševním zdravím jeví v ČR jako nedostatečná, což vede k podcenění péče o pacienty s těmito potížemi. Zavedení rutinního screeningu na přítomnost disociativních symptomů v klinické praxi (například pomocí dotazníku A-DES, který byl vyvinut pro zjišťování disociativní symptomatologie), by mohlo přispět ke zlepšení stávající neutěšené situace.

Abstract:

The dissertation thesis consists of ten studies dealing with the history of trauma and dissociative experience in girls. The individual texts reflect the author’s more than 20-year-long experience with psychiatric and psychotherapeutical work, incorporating both clinical and research topics. The thesis includes review articles, case studies, qualitative text analyses and quantitative research of the associations between sexual trauma, dissociation and suicidal tendencies. One of the most important outcomes relates to findings about the far-reaching impacts of early childhood trauma on the development of psychopathology both in childhood and adulthood, including the impact of trauma on lifelong physical health. There is a direct connection between early trauma, especially sexual abuse of girls, and the development of dissociative experience. The author and her co-authors found that traumatized adolescent girls are prone to self-injury, suicidal ideation and suicidal attempts. The results of the mediation analysis suggest that girls with sexual abuse history have suicidal tendencies especially when dissociative symptomatology co-occurs. After dissociative disorder symptoms are identified, the patient should receive intensive psychotherapeutical treatment, as there is a high risk of self-injurious or suicidal tendencies. The psychotherapy of traumatized individuals should focus on the reduction of dissociative experience. It has been proved that the efficient specialist procedures include the externalization of the difficulties, using expressive, imaginative and metaphorical expressions which enable to contain unbearable, unimaginable and sometimes unspeakable experience associated with trauma. In such ways the traumatized individual can seek the necessary change and start the healing process. In the Czech Republic the overall insight of mental health professionals into dissociative disorders appears to be insufficient, which makes them underestimate the appropriate care in patients suffering from such problems. The implementation of routine screening for the presence of dissociative symptoms in clinical practice (e.g. using the A-DES questionnaire that was developed to test for dissociative symptomatology) could improve the current unsatisfactory practice.
