The retention of transpedicular screws in osteoporotic vertebrae

Abstract:

Stabilizační operace páteře patří dnes k nejčastějším spondylochirurgickým výkonům. Pro ukotvení montáže do obratlů jsou nejčastěji používány transpedikulární šrouby. S postupním stárnutím populace, a tedy i operovaných pacientů, operujeme stále častěji pacienty s osteoporózou či osteopenií, u kterých se riziko uvolnění montáže zvyšuje. Do studie bylo zařazeno 50 pacientů, u kterých byla v letech 2015-2017 provedena stabilizace páteře pro degenerativní postižení. Vyhodnoceny byly pomocí neparametrických testů (Kruskal-Wallisova, Mann-Whitneyho a Wilcoxonova testu) rentgenologické spinopelvické parametry a klinické parametry. V samostatném experimentu jsme hodnotili pull-out sílu nutnou k dislokaci 4 typů transpedikulárních šroubů implantovaných do prasečích obratlů. Výsledky byly hodnoceny pomocí ANOVA testu s post hoc analýzou. U 38 z 50 pacientů, u kterých na konci sledování nebyl PI-LL mismatch, tedy PI-LL bylo ≤10°, byl proti zbytku souboru statisticky významný rozdíl v pelvic tilt (p=0,049) a sagittal vertical axis (p<0,001); klaudikace, VAS a OSWESTRY byly bez statisticky významného rozdílu. Porovnávali jsme také rozdíly podle rozsahu a typu stabilizace. Významná změna byla v parametru klaudikací 12 a 24 měsíců po operaci (p=0,007, p=0,005) ve prospěch 360° fúze proti zadní fúzi. VAS a ODI bylo v obou skupinách v čase zlepšeno (z 62 na 32, respektive z 62 na 30) také statisticky významně (p<0,001 v obou skupinách), porovnání skupin proti sobě vyšlo statisticky nevýznamné. Z 238 implantovaných šroubů došlo k dislokaci pouze u 3 šroubů a u dalších 2 šroubů se objevilo halo v RTG obraze (5 šroubů z 238, 2%). Vzhledem k minimálnímu počtu uvolněných šroubů jsme tyto statisticky nehodnotili. V experimentální části nebyl prokázán statisticky významný rozdíl v pull-out testu mezi šrouby s průměrem 5 a 6mm (Favg = 898±189; 1073±176), ani mezi šrouby augmentovanými a expandibilními (Favg = 1610±192; 1704±101). Statisticky významný rozdíl na hladině p=0,05 byl však prokázán mezi 6mm šrouby a šrouby expandibilními i augmentovanými (Favg= 1073±176; 1704±101; 1610±192), stejně jako mezi 5mm šrouby a šrouby expandibilními i augmentovanými (Favg = 898±189; 1704±101; 1610±192). Korektní spinopelvické balance páteře u pacientů po operaci páteře je velmi důležitým ukazatelem pooperačního vývoje a stavu, v našem souboru jsme ale nepozorovali statisticky významný rozdíl v klinických výsledcích u pacientů, u kterých nebyly sagitální parametry pooperačně optimální. Použitím augmentovaných či expandibilních šroubů můžeme významně zvýšit retenci šroubů v obratli a tím stabilitu celé montáže.

Abstract:

Spinal stabilisation is nowadays one of the most common spinal surgeries. Pedicular screws are the most frequently employed method of assembly fixation. The population ageing is associated with increasing numbers of patients with osteopenia or osteoporosis and, in effect, with a higher risk of assembly loosening. 50 patients who underwent spinal stabilisation due to degenerative changes between 2015 and 2017 were included in the study. X-ray spinopelvic and clinical parameters were evaluated using non-parametric Kruskal-Wallis, Mann-Whitney and Wilcoxon tests. Experimentally, pull-out strength necessary for dislocation of 4 types of pedicular screws implanted into porcine vertebrae was investigated; the results were assessed using ANOVA test with post hoc analysis. There was a statistically significant difference in pelvic tilt (p=0.049) and sagittal vertical axis (p<0.001) between the patients in whom PI-LL mismatch was not observed by the end of the observation period (38 out of 50) and those in whom it was observed (12 out of 50). No statistically significant difference was recorded in claudications, VAS and OSWESTRY. We have also compared the differences in the extent and type of stabilisation. A significant difference favouring 360o fusion over the posterior fusion was detected in the parameters „claudications 12/24 months after surgery“ (p=0.007 and p=0.005, respectively). VAS and ODI statistically significantly improved over time in both groups (from 62 to 32 and to 30, respectively; p<0.001 in each group); mutual comparison of the groups however did not reveal any significant differences. Only 5 out of 238 implanted screws (2%) were problematic (dislocation in three screws and a halo on X-ray in 2 more screws), which prevented statistical evaluation. The performed experiment did not reveal any differences in the pull-out strength neither between screws with 5 mm and 6 mm diameter (Favg = 898±190 and 1073±176, respectively) nor between augmented and expandable screws (Favg = 1610±192 and 1704±101, respectively). A statistically significant difference (p<0.05) was detected between 6 mm screws and expandable as well as augmented screws (Favg=1073±176; 1704±101; 1610±193), and between 5 mm screws and both expandable and augmented screws (Favg = 898±190; 1704±101; 1610±193). The correct spinopelvic balance in patients after spinal surgery is an important indicator of post-surgery development and patient condition. In our patient group, we have, however, not found any statistically significant difference in clinical outcomes in patients whose sagittal parameters were not optimal after the surgery. The use of augmented or expandable screws significantly increases screw retention in the vertebrae and, therefore, the stability of the entire assembly.
