Diagnosis of mucous membrane pemphigoid by Western blot

Abstract:

Diplomová práce si klade za cíl zpřesnit diferenciální diagnostiku bulózních dermatitid, především diagnostiku jizvícího slizničního pemfigoidu. Je zaměřena na studium reaktivity autoprotilátek obsažených v sérech pacientů metodou Western blotu s důrazem na reaktivitu proti proteinům lamininu 332 a integrinu α6β4. Substrátem pro analýzu Western blotem byly buňky a ECM z linie lidských keratinocytů HaCaT. Byla vypracována metoda izolace a lýzy extracelulární matrix keratinocytů a optimalizován postup tvorby lyzátu celých buněk. Výsledky analýz byly porovnány s klinickými projevy pacientů a byly nalezeny antigeny vhodné pro další zkoumání. Konkrétně jde o neidentifikované, ale u pacientů časté antigeny o velikostech 60 kDa a 70 kDa, antigen 120 kDa u pacientů s očními projevy a možný fragment lamininu 332 o 110 kDa. Přínosná by mohla být také další analýza fragmentů o nízkých molekulových hmotnostech okolo 40 kDa, které jsou u pacientů velice často pozitivní.

Abstract:

The diploma thesis sets out to improve the differential diagnostics of bullous dermatoses, especially the diagnostics of mucous membrane pemphigoid. It aims to study the reactivity of autoantibodies contained in patients’ sera by Western blotting with a special interest in the reactivity to the proteins laminin 332 and integrin α6β4. The substrates for the Western blot analyses were cell and ECM lysates from the human keratinocyte line HaCaT. A method to extract and lyse the extracellular matrix of the keratinocytes was created and the process to obtain a whole-cell lysate was optimised. The results of the analyses were compared to the patients’ clinical symptoms and antigens suitable for further research were determined. Specifically, these are the unidentified antigens sized around 60 kDa and 70 kDa commonly found in patients, the 120 kDa antigen found in patients with ocular symptoms, and a possible laminin 332 fragment sized 110 kDa. Further analysis of low molecular weight fragments at around 40 kDa, which are very commonly positive in patients, might also be beneficial.
