Epidemiology of the fifth and sixth diseases in the Pardubice region

Abstract:

Pátá a šestá nemoc patří mezi méně známá infekční exantémová onemocnění, projevující se kožní vyrážkou a běžnými příznaky nachlazení. Jejich výskyt je celosvětový a postihují především dětskou populaci. Cílem této bakalářské práce je v teoretické části objasnit historický vývoj páté a šesté nemoci, dále jejich etiologii, patogenezi, epidemiologii, klinický obraz, diagnostiku, terapii a v neposlední řadě také prevenci těchto dvou onemocnění. Dále je cílem v praktické části analyzovat a vyhodnotit data ohledně výskytu páté a šesté nemoci v Pardubickém kraji během let 2008-2017 a následně provést komparaci s výskytem v České republice. Získaná data byla rovněž analyzována podle počtu případů dle pohlaví, věku a sezónnosti výskytu. Mimo jiné byl zkoumán podíl hlášených případů v jednotlivých okresech Pardubického kraje. Výsledky byly následně zpracovány formou přehledných grafů a tabulek, které vyobrazují shromážděná data. Během let 2008-2017 bylo v celé České republice nahlášeno celkem 24 041 případů pod diagnózou B08, jenž zahrnuje pátou a šestou nemoc společně i s jinými infekčními onemocněními, čímž byla data zkreslena. Porovnáním počtu hlášených případů diagnózy B08 v Pardubickém kraji a České republice byla zjištěna skutečnost, že nemocnost v České republice je vyšší, až na rok 2017, kdy v počtu hlášených případů dominoval Pardubický kraj. Analýzou jednotlivých okresů Pardubického kraje bylo zjištěno, že okres Svitavy byl diagnózou B08 zasažen nejvíce, zejména roku 2017, kdy bylo v daném okrese zaznamenáno celkem 172 případů, což byl nejvyšší počet za celé sledované období v Pardubickém kraji. V poskytnutých datech došlo od roku 2013 k rozlišení jednotlivých diagnóz v Pardubickém kraji, proto bylo možné provést podrobnější analýzu dat. Během let 2013-2017 bylo v Pardubickém kraji zaznamenáno 124 případů páté nemoci a 125 případů nemoci šesté, lze tedy s jistotou konstatovat, že výskyt obou onemocnění byl ve sledovaném období téměř totožný. Rovněž byla ověřena skutečnost, že výše zmíněná onemocnění se vyskytují zejména u dětí. Pátá nemoc dominovala především u dětí ve věkovém rozmezí 5-9 let (57 %), zatímco nemoc šestá převažovala nejvíce u věkové skupiny dětí do 1 roku (56 %), což je mimo jiné uváděno i v odborné literatuře zabývající se danou problematikou.

Abstract:

The fifth and sixth diseases are lesser-known infectious exanthematous diseases, manifested by a skin rash and common cold symptoms. Their incidence is worldwide and they mainly affect the child population. The aim of this bachelor thesis is to explain the historical development of the fifth and sixth diseases, their etiology, pathogenesis, epidemiology, clinical manifestation, diagnosis, therapy and, last but not least, prevention of these two diseases. Furthermore, the aim of the practical part is to analyze and evaluate the data on the incidence of the fifth and sixth diseases in the Pardubice Region during 2008-2017 and then to compare it with the incidence in the Czech Republic. The data obtained were also analysed according to the number of cases by sex, age and seasonality of occurrence. Among other things, the proportion of reported cases in each district of the Pardubice Region was examined. The results were processed in the form of synoptical graphs and tables showing the collected data. During 2008-2017, a total of 24,041 cases were reported across the Czech Republic under the diagnosis B08, which includes the fifth and sixth diseases along with other infectious diseases. A comparison of the number of reported cases of B08 in the Pardubice Region and the Czech Republic revealed that the incidence in the Czech Republic is higher except for 2017, when the Pardubice Region dominated in the number of reported cases. The analysis of the individual districts of the Pardubice Region showed that the Svitavy district was the most affected by B08, especially in 2017, when a total of 172 cases were reported in the district, which was the highest in the whole period under review in the Pardubice Region. In the data provided, there has been a breakdown of the individual diagnoses in the Pardubice Region since 2013, so a more detailed analysis of the data was possible. During the years 2013-2017, 124 cases of the fifth disease and 125 cases of the sixth disease were recorded in the Pardubice Region, so it can be stated with certainty that the incidence of both diseases was almost identical during the period under study. It was also verified that the above-mentioned diseases occur mainly in children. Fifth disease was predominant in children aged 5-9 years (57 %), while sixth disease was most prevalent in the age group of children under 1 year (56 %), which is also reported in the literature on the subject.
