Nursing process in care of patient after aortic valve implantation by the TAVI method

Abstract:

Cílem bakalářské práce je vytvořit ošetřovatelský proces u pacienta po implantaci aortální chlopně metodou TAVI. Pojem TAVI označuje transkatetrizační implantaci aortální chlopně. Tento trend se v kardiologii provádí už cca 20 let a vyspěl v metodu, která umožňuje pacientům s vysokým rizikem podstoupit náhradu aortální chlopně jako miniinvazivní alternativu otevřené operace, která pro ně představuje riziko. Práce je rozdělena na teoretickou a empirickou část. Teoretická část je rozdělena na tři kapitoly. V první kapitole je popsána aortální stenóza, a to konkrétně její etiologie a patofyziologie, klinický obraz, diagnostika a možnosti terapie tohoto onemocnění. Druhá kapitola je věnována charakteristice TAVI, indikacím a kontraindikacím k výkonu, komplikacím po výkonu, popisu chlopní. Ve třetí kapitole jsou shrnuta specifika ošetřovatelské péče prováděná sestrou u pacienta po výkonu na jednotce intenzivní péče a následně na standardním oddělení, kam je pacient posléze přeložen. V empirické části práce byl vytvořen ošetřovatelský proces. U pacientky byla odebrána ošetřovatelská anamnéza při přijetí na jednotce intenzivní péče po výkonu, podle které byly následně stanoveny ošetřovatelské diagnózy dle NANDA-I Taxonomie II. U pacientky na základě získaných dat bylo po výkonu stanoveno 25 ošetřovatelských diagnóz z nichž je 13 diagnóz aktuálních a 12 diagnóz potencionálních. K pěti ošetřovatelským diagnózám byl stanovený cíl, byly vytvořeny intervence, které byly následně zrealizovány a zhodnoceny. Cíle ošetřovatelské péče vztahující se k ošetřovatelským diagnózám byly dle hodnocení v této práci částečně nebo plně splněny.

Abstract:

The aim of the bachelor thesis is to create a nursing process in care of patient afteraortic valve implantation by the TAVI method. The term TAVI refers to transcatheter aortic valve implantation. This has been performed in cardiology for approximately 20 years and has evolved into a method that allows high-risk patients to undergo aortic valve replacement as a minimally invasive alternative to open surgery. The thesis is divided into theoretical and empirical parts. The theoretical part is divided into three chapters. The first chapter describes aortic stenosis, specifically its etiology and pathophysiology, clinical picture, diagnosis and therapeutic options for this disease. The second chapter is devoted to the characteristics of TAVI, indications and contraindications to the procedure, complications after the procedure, and a description of the valves. The third chapter summarizes the specifics of nursing care performed by the nurse on the patient after the procedure in the intensive care unit and then in the standard ward where the patient is subsequently transferred. In the empirical part of the thesis, the nursing process was create. The nursing assessment of the patient was obtained after admission to the intensive care unit after surgery, afterwards nursing diagnosis according to NANDA-I Taxonomy II were determined. Based on the data obtained, the patient was given 25 nursing diagnoses after the procedure, of which 13 were current diagnoses and 12 were potential diagnoses. For the five nursing diagnoses, a goal was set, interventions were developed, which were subsequently implemented and evaluated. According to the evaluation in the thesis, the goals of nursing care related to nursing diagnosis were partially or fully met.
