Molecular pathogenesis of chronic lymphocytic leukemia

Abstract:

The goal of this thesis is to summarize the knowledge of molecular landscape of chronic lymphocytic leukemia (CLL). It focuses on deregulated pathological and physiological processes that play crucial role in this so far incurable disease. Altogether it sums up their influence on the origin, progress and prognosis of the disease. The thesis describes genetic aberations del(17)(p13), del(11)(q22-23), trisomy of chromosome 12 and del(13)(q14), that target larger parts of the genome, as well as specific mutations in particular genes NOTCH1, SF3B1 or BIRC3. Deregulation of microRNA levels that are important in the CLL pathogenesis is also partly discussed. The thesis then summarizes the interactions of malignant B-cells in the microenvironment of bone marrow and secondary lymphoid organs without whom the malignant B-cells would die of apoptosis. The signalization through the B-cell receptor whose signalling pathway plays the key role in the survival of B-cells is illustrated in detail. The end of the thesis deals with mechanisms of novel therapeutic agents that are based on the understanding of the molecular pathways described above.

Abstract:

Cílem této bakalářské práce je přehledně shrnout dosud známe poznatky o molekulární podstatě chronické lymfocytární leukémie (CLL). Zaměřuje se na patologické a fyziologické procesy, jejichž deregulace stojí za vznikem, progresí a prognózou tohoto dosud nevyléčitelného onemocnění. Práce popisuje genetické aberace del(17)(p13), del(11)(q22-23), trizomii chromozomu 12 a del(13)(q14), které postihují větší části genomu, a taktéž specifické mutace v konkrétních genech NOTCH1, SF3B1 či BIRC3. V patogenezi CLL hraje roli i deregulace hladin microRNA, jejichž význam je také částečně diskutován. Práce dále popisuje interakce nádorových buněk v mikroprostředí kostní dřeně a sekudárních lymfatických orgánů, bez kterých by maligní B-buňky podléhaly apoptóze. Detailněji se práce věnuje signalizaci prěs B-buněčný receptor, jehož signální dráha je pro přežití CLL buněk klíčová. Na závěr jsou představeny mechanizmy působení nových léčiv založených právě na detailním pochopení molekulárních drah diskutovaných výše.
