Epidemiology of lung cancer in the Czech Republic

Abstract:

In this master thesis, we deal with lung cancer, which has been a significant burden for the Czech population for a long time. As this cancer occupies the first positions among malignant neoplasms, both in the case of incidence and mortality, the National Screening Center of the Czech Republic is working on a pilot screening study in the Czech Republic. Due to the potential introduction of a screening program, this master thesis aims to perform a comprehensive analysis of the epidemiology of lung cancer. This work firstly describes epidemiological indicators, then it describes lung cancer with respect to individual types of classification and its etiopathogenesis. The theoretical part, among other things, includes a complete epidemiological overview of lung cancer, screening and selected screening studies of carcinoma together with a comprehensive overview of statistical methods used for analysis in the practical part. Namely, it is a survival analysis, joinpoint regression and age-period-cohort models. The practical part begins with a detailed methodological description of survival analysis, joinpoint regression and age-period-cohort models together with an introduction of used data sources. Survival analysis has shown that half of the patients pass away within one year after the diagnosis. With regards to survival, selected patient characteristics were also discussed in this section. Joinpoint regression showed a different development of the epidemiological indicators between men and women. Joinpoint regression was also applied to the population of men and women divided by age and morphological type, which confirmed that different development of the trend in men and women also depends on individual subcategories. Furthermore, using age-period-cohort models, it was found out that the risk of developing cancer together with the risk of dying from this cancer is significantly dependent on age at diagnosis of disease (death), year at the diagnosis (death) and year of birth.

Abstract:

V tejto diplomovej práci sa venujeme bronchogénnemu karcinómu, ktorý dlhodobo predstavuje podstatnú záťaž pre českú populáciu. Keďže tento karcinóm obsadzuje prvé priečky medzi zhubnými novotvarmi ako v prípade incidencie, tak aj mortality, pracuje Národné skríningové centrum ČR na pilotnej skríningovej štúdii v Českej republike. Vzhľadom na potenciálne zavedenie skríningového programu je cieľom tejto diplomovej práce vykonanie komplexnej analýzy epidemiológie bronchogénneho karcinómu. Táto práca prináša v teoretickej časti popis epidemiologických ukazovateľov a bronchogénneho karcinómu vzhľadom na jednotlivé typy klasifikácie a jeho etiopatogenézu. V teoretickej časti je taktiež zakomponovaný kompletný epidemiologický prehľad bronchogénneho karcinómu, skríning a vybrané skríningové štúdie karcinómu spolu s uceleným prehľadom štatistických metód využívaných k analýze v praktickej časti. Menovite ide o analýzu prežívania, joinpoint regresiu a age-period-cohort modely. Praktická časť začína podrobným metodickým popisom analýzy prežívania, joinpoint regresie a age-period-cohort modelov spolu s predstavením využívaných dátových zdrojov. Pomocou analýzy prežívania bolo ukázané, že polovica pacientov sa od diagnózy nedožije ani jedného roku. Vzhľadom na prežívanie boli v tejto časti taktiež prediskutované vybrané charakteristiky pacientov. Joinpoint regresia ukázala odlišný vývoj epidemiologických ukazovateľov medzi mužmi a ženami. Joinpoint regresia bola taktiež aplikovaná na populáciu mužov a žien rozdelenú podľa veku a morfologického typu, čo potvrdilo, že rozdielny vývoj trendu u mužov a žien taktiež závisí od jednotlivých podkategórií. Ďalej bolo pomocou age-period-cohort modelov zistené, že riziko vzniku karcinómu spolu s rizikom úmrtia na tento karcinóm je významne závislé na veku pri diagnóze (úmrtí), roku diagnózy ochorenia (úmrtia) a roku narodenia.
