Mitotická aktivita a proliferační index u difúzního velkobuněčného B-lymfomu jako diagnostický faktor onemocnění

Mitotic activity and proliferation index of diffuse large B-cell lymphoma as a diagnostic factor of the disease

Abstract:

Bakalářská práce se zabývá problematikou diagnostiky lymfoproliferativních onemocnění, a to konkrétně nejčastějšího lymfomu - difúzního velkobuněčného B-lymfomu (DLBCL) ve vztahu k mitotické aktivitě a proliferačnímu indexu Ki-67. Teoretická část bakalářské práce se snaží čtenáři nastínit anatomii a fyziologii samotného lymfatického systému a popsat základní pojmy a souvislosti. Následně se téma zaměřuje na lymfatické uzliny (nodi lymphatici) a jejich patologii. Zde jsou popsány možné regresivní změny lymfatických uzlin, lymfadenitidy, lymfadenopatie, a především nádorová onemocnění. Další kapitola se zabývá právě patologií lymfoproliferativních onemocnění a jejich rozdělením, následně jsou konkrétně popsány obecné charakteristiky Hodgkinových a Non-Hodgkinových lymfomů. Poslední a klíčová kapitola teoretické části bakalářské práce pojednává čistě o difúzním velkobuněčném B-lymfomu, jeho charakteristice, rozdělení, genetických vlastnostech, diagnostice, imunohistochemickém vyšetření a prognóze. Experimentální část pracuje se souborem pacientů (n = 52) diagnostikovaných v CGB laboratoře a.s. v letech 2010-2019 s diagnózou DLBCL. U každého pacienta byl mikroskopickým vyšetřením stanoven mitotický index a proliferační index Ki-67. Získaná data byla zkoumána statistickým testem korelace v softwaru R. Pomocí korelačního koecifientu, Spearmanova nebo Pearsonova korelačního koeficientu a p-value byly zodpovězeny otázky, zda mezi proměnnými existuje korelace. Korelační test potvrdil, že mezi diagnosticko-prognostickými faktory proliferačním indexem Ki-67 a mitotickým indexem existuje silná korelační závislost (p-value = 5,87×10-11). Tato korelace byla u žen (p-value = 5,46×10-11) výraznější, než u mužů (p-value = 0,0047). Naopak se neprokázala korelace mezi proliferačním indexem Ki-67 a věkem pacientů (p-value = 0,3381) ani mezi mitotickým indexem a věkem pacientů (p = 0,1870).

Abstract:

The bachelor thesis deals with the diagnosis of lymphoproliferative disease, specifically the most common lymphoma - diffuse large B-cell lymphoma (DLBCL) in relation with mitotic activity and proliferation index Ki-67. The theoretical part of the thesis tries to approximate the anatomy and physiology of the lymphatic system and describe the basic concepts and contexts. Consequently, the topic focuses on lymphatic nodes (nodi lymphatici) and their pathology. There are described possible regressive changes, lymphadenitis, lymphadenopathy, and especially cancer. The next chapter deals with pathology of lymphoproliferative diseases and their distribution, followed by a specific description of the general characteristic of Hodgkin's and Non-Hodgkin's lymphomas. The last and the key chapter of the theoretical part of the thesis is about diffuse large B-cell lymphoma, its characteristics, genetic properties, diagnosis, immunohistochemical examination and prognosis. The experimental part works with a group of patients (n = 52) diagnosed in CGB laboratory a.s. between 2010 - 2019 with the diagnosis of DLBCL. The mitotic index and the proliferation index Ki-67 were determined by microscopic examination in each patient. The obtained data were examined by statistical test of correlation in software R. The questions if there is a correlation between variables were answered by correlation coefficient, Spearman's or Pearson's correlation coefficient and p-value. The correlation test confirmed that there is a strong correlation (p-value = 5,87×10-11) between the diagnostic-prognostic factors of proliferation index Ki-67 and mitotic index. This correlation was stronger in women's case (p-value = 5,46×10-11), than in men's case (p-value = 0,0047). On the other hand, no correlation was found between the proliferation index Ki-67 and the age of the patients (p-value = 0,3381) neither between the mitotic index and the age of the patients (p-value = 0,1870).
