Therapeutic use of the insoles in the patient with Achillodynia

Abstract:

Úvod: Téma práce je terapeutické využití vložek do bot u pacienta s Achillodynií. Cíl práce: Je rešeršní zpracování problematiky, týkající terapeutického využití vložek do bot u pacientů s Achillodynií, seznámení s klinickými projevy Achillodynie s četností výskytu, diagnostikou tohoto onemocnění a možností komplexní terapie s využitím vložek do bot jako terapeutického prostředku. Metodika: V této práci je zpracována kazuistika pacienta s diagnózou Achillodynie. Anamnéza byla odebrána na základě vstupního rozhovoru. Součástí práce je vstupní a výstupní kineziologický rozbor a speciální testy. Na základě vyšetření byl sestaven krátkodobý fyzioterapeutický plán. Fyzioterapeutická intervence trvala 8 týdnů. V polovině terapie bylo provedeno kontrolní hodnocení. Během 15 návštěv byly využity různé koncepty a metody: mobilizace, TMT, PNF, ACT, senzomotorika, Spiraldynamik?. Součástí terapie byla i fyzikální terapie a také dlouhodobé nošení individuálně vyrobených vložky do bot. Po 2 měsících terapie byl proveden výstupní kineziologický rozbor a na jeho základě navržen dlouhodobý rehabilitační plán. Výsledky: Zjištěné subjektivní a objektivní změny po skončení terapie byly porovnány. Nastaly pozitivní změny v ústupu bolesti, odstranění otoku, uvolnění měkkých tkání a zejména zkrácených struktur. Zlepšení nastalo v centrovaném postavení nohy a v její mobilitě a v jejím zapojení do svalových řetězců a pohybových stereotypů. Zlepšilo se osové postavení a spirální sešroubování dolních končetin a postavení pánve. Došlo ke zlepšení hybnosti páteře což je podloženo goniometrickým měřením. Zlepšen byl stereotyp chůze, zkvalitnilo se brániční dýchání. Všechny nové pohybové návyky se podařilo propojit do běžných denních aktivit. Závěr: Zlepšení celkového stavu se stalo pro pacientka motivací pro dlouhodobější změnu, aktivní přístup a změnu pohybových návyků. Pro mě se tato práce stala přínosem pro další praxi a obohatila mě o nové poznatky. Aktivní fyzioterapie a kombinace různých fyzioterapeutických přístupů a konceptů s aplikací vložek do bot přispěla k řešení problému, který pacient měl.

Abstract:

Introduction: The topic of this work is the therapeutic use of insoles for patients with Achillodynia. Aim of the work: To provide an overview of the problem of therapeutic use of insoles for patients with Achillodynia, to present clinical manifestations of Achillodynia with frequency of occurence, diagnosis of this disease and the possibility of comprehensive therapy using insoles as a therapeutic agent. Methods: : In this work a case report of a patient with the diagnosis of Achillodynia is processed. The history was taken based on an initial interview. The work includes input and output kinesiological analysis and special tests. Based on the examination, a short-term physiotherapy plan was drawn up. The physiotherapeutic intervention lasted 8 weeks. A mid-term review was performed. During the 15 visits, different concepts and methods were used: mobilization, TMT, PNF, ACT, sensorimotor, Spiraldynamik?. The therapy also included physical therapy and long-term wearing of individually manufactured shoe insoles. After 2 months of therapy, an outcome kinesiological analysis was performed and a long-term rehabilitation plan was proposed. Results: The observed subjective and objective changes after the end of therapy were compared. There have been positive changes in pain relief, swelling removal, soft tissue relaxation and especially shortened structures. Improvement occurred in the centered position of the foot and in its mobility and in its involvement in muscle chains and movement stereotypes. Improved axial positioning and spiral screwing of lower legs and pelvis positioning. The spine momentum was improved, which is supported by trigonometric measurements. The stereotype of walking was improved, diaphragmatic breathing improved. We managed to integrate all new physical habits into daily activities. Conclusion: Improvement of the overall condition became a motivation for the patient for a long-term change, active approach and change of movement habits. For me, this work has become an asset for further practice and has enriched me with new knowledge. Active physiotherapy and a combination of different physiotherapeutic approaches and concepts with the application of insoles contributed to solving the problem the patient had.
